Mahal received her St. Lukes physical exam 4 months ago in Manila. The results are valid for one year. We have the vaccinations record, DS-3025, and she needs just 3 more Vaccinations.

We have the Chest X-ray CD as well. But we do not have any documents regarding her physical exam, which she passed just fine.

Does USCIS have those physical exam results on file when we submit the AOS packet, or is it needed to have the civil surgeon complete another physical exam, and complete the required vaccinations?

If I find a fair-priced civil surgeon in San Diego, I will add it to the VJ list.

My question boils down to must we pay for another physical exam at this point? Is it better just to have the civil surgeon do the whole medical work up for AOS inspite of the physical exam 4 months ago?

Foreign medical exam is valid for up to one year for residency application ... however the vaccination component may or may not be

But we did not receive the documentation for the physical exam results. I think those were in the sealed yellow envelope surrendered at POE.

So I don't know how the civil surgeon would be able to review the physical exam?

I'm thinking I will just have to bite the bullet and pay for another exam in order to have kosher documentation for AOS.

But If we really don't have to do this, I hope to find out how.

Thanks.

Civil surgeon would not review the exam but complete a brand new one if you went that way... When you file for AOS, they request your "A" file and in it should be all the documents that you submitted to the POE... and thus the medical exam will (or should be) there. We did not do a new exam for AOS as they used the foreign medical (DS-2053) instead that was in the POE packet.

I would not "bite the bullet" unless you are RFE'd in the event of a misfiled exam.

OK, great to know that USICS gets the 'A' file and has those physical exam results.

Problem is, many civil surgeons "Won't complete another doctor's work" the secretary says on the phone, so they won't just transcribe/complete your vaccination record and give you the sealed envelope.

I found one civil surgeon who performs the full exam for $95, so i will have that as a plan B.

AOS is a bear, but at least we are together and not an ocean apart while completing it.

There is a CS in San Diego/Mira Mesa that will transcribe the vacanations, and put in in a sealed envelope for $25 USD. Contact info is Dr. Annabelle Razon-San Angel 9190 Mira Mesa Blvd San Diego, CA 92126 Tel: (858)689-1814. I had several others insist on the full exam.

A new physical exam is not necessary nor is it required. Many civil surgeons do not realize this.

Simply find a civil surgeon who understands AOS, will transcibe the vaccination record, and will seal it in the envelope.

It took us several phone calls to civil surgeons before we found one who knew the drill. It took the civil surgeon about 15 minutes to do the job, and it cost us about $30-40.

On a side note...at our AOS interview, the interviewer had on his desk what looked to be our complete K1 packet. It appears that the USEM ships the entire packet to the local DHS office.

Ok here is what I think most people and Civil Surgeons are missing. If you follow the link above to USCIS for the Q + A on TB skin testing and vaccination requirements, you will see the following Q + A.

Q: If an applicant is required to receive only the vaccinations, and not undergo the entire medical exam, does he/she have to submit the entire Form I-693?

No. Applicants who are not required to have the entire medical examination need to submit only pages one, three and five of the new form. The required sections include Part 1, Information About You, the vaccination record portion of Part 2, and Part 5 (if the vaccinations are administered by a civil surgeon) or Part 6 (if the vaccinations are administered by a local health department). Pages two and four, that would have been left blank, do not need to be submitted.

Because the last page of the I-693 (where the doctor signs) says that he certifies that he , the CS, did a med. exam, he may not want to sign it without him doing an exam, thus charging you a fee for another exam. Copy the entire Q+A and bring it with you to convince the CS you will not need another exam regardless of any other immunizations or TB skin test. Good luck! Mari-Joe

So very true. Most CS do not know what they are supposed to do and are in it to make a killing.

In my case, I found a Civil Surgeon in Portland, Dr. Al Torres, who transcribed my and my son's medical record for the unbelievable amount of $10 (!!!) each in 2007. The good doctor did not engage in gouging his patients, tried to speak Tagalog to me, took the time to photocopy an article he was reading about Dr. Jose Rizal (our national hero) to give to me and even sang me a song! It was a totally awesome experience with him. I wish all doctors were like him.

Before I went to the Civil Surgeon's office I printed out from VisaJourney and also from the PDF I-485 instructions detailing exactly what was needed for a K1 with an exam within the last year (and a copy of the I-693 with personal info. filled in), but did not need at the clinic I went to, they were familiar with the requirements. It was a Concentra clinic, but I don't see any of those on their locator super close to you.

I didn't call any, I thought I'll have a better chance with paper in hand explaining exactly what I needed.

The brown/ yellow packet IS containing your medical exam. It's not a should, it is.

I paid $40 for a transcription only, and mailed my AOS today.
