I know I'm late to this thread, but I had a difficult time finding a civil surgeon in Connecticut who didn't insist I needed the full medical exam again, even though my medical exam results are still in date. I finally found a doctor in Stamford who will transcribe the vaccinations for me for $50 so thought I'd share their name on here and add to the existing list:

It's in Salem, and hour's drive south of Portland. But, after days of calling the USCIS Civil Surgeon List (mostly out-of-date, and most will only do a full medical), found this clinic in Salem: $20 to complete the relevant vaccinations of the I-693, plus the cost of the two additional vaccinations I need.

Dr Torres' clinic in Portland is temporarily closed. But worth calling to see if he opens up again in the future:

Information on the Portland, Oregon area was lacking in this thread. A separate thread has been merged into the main thread, and the posts in that separate thread have been edited in order to reveal relevant information here.

The first post that follows was from yet another Portland-related thread, from 30 Jan 2014 (current accuracy unknown):

I had all my vaccinations done at my regular doctors, and then went to see Dr F. Douglas Day, who charged $50 to check all the forms, transcribe my UK vaccination record, and the ones i had when i arrived in the US, to the I-693 form, then sign it all off for $50. I took all forms in, was asked to come back in 30 mins, and it was ready and waiting for me to collect. Very efficient, i think it was worth spending the money for piece of mind. The receptionist is very helpful!

They also offer a full range of vaccinations, but mostly i found it was cheaper to go to a pharmacy or a family doctors clinic for those.

Hiya - if anyone's looking to get additional vaccinations done & relevant vaccinations sections of the I-693 completed, or they've had the vaccinations have medical proof and just need a civil surgeon to confirm on the I-693....

I ended up calling the list until found this clinic in Salem - it's an hour's drive away but worth the $$$ saving. They charge $20 to complete the paperwork plus the costs of any vaccinations required:

I talked to Dr A. Torres and he told me he had a terrible accident and he is not working to USCIS anymore at least so far.

I got my vaccination report in Salem and the Dr. Howard Tsang only charged $20. It was fast.

I have to reply to you, because you saved me a lot of money! Thank you so much for posting on here!

I got an RFE (post interview) for not having my varicella shot (I guess it was missed in London and VH wasn't listed on my vaccination document, as I had chicken pox as a child and told the doctor there that). I originally called a doctor in Bridgeport who wanted to charge $450 for a full medical (not required with K-1 if you have filed your I-485 within one year).

Called Marietta, she was great and I got in for an appointment within a couple of days. She charged $50 for the vaccination record update, and $50 for the varicella titer, and $25 for an office visit. $125 total and I was done within 20minutes.

They don't mail results, so just allow some time to come and pick everything up once the results are back and mail onto USCIS.

I am very pleased with Concentra Medical Center in Akron, OH. I went to this particular place, based on a few positive reviews I found on this site and I wasn't disappointed!

The appointment was set up by my husband, he experience some trouble explaining to people at the reception that I don't need a full medical exam, just the vaccination chart (AOS from K-1), they seemed clueless, but once he got to talk with Dr. Benson, the conversation went smoothly, he was very helpful and quickly verified the fee (a reasonable $43).

On the appointment day, the receptionist struggled yet again to understand what I wanted so she made me fill out and sign a tone of forms necessary for the full exam. Once I told her I didn't need to undergo a medical exam, she said it would be the doctor that would make the final call... Luckily, once I was called by a nurse, after an initial vision and blood pressure tests, she called Dr Benson. He is such a nice and friendly person! He apologized at least twice that I had to complete so many useless forms and then quickly filled out my vaccination chart. Since it was the beginning of October he gave me a flu shot ($25) and afterwards, I was given a sealed envelop + a copy for my records.

A very nice experience once you get to talk to the right person. A piece of advice for everyone: make the reception staff call Dr. Benson immediately once you show up to avoid the paperwork .

I just want to share that we went to Dr Benson recently, and same as the poster above, we had a good experience with him and Concentra Medical Center. I called the number listed and one of the nurses I spoke to was familiar with the process for AOS. On our appointment, I was just asked to fill out a some paperwork and then met with Dr Benson afterwards. He asked me a few questions and recommended I get a flu shot since it's flu season.

Upon payment, one of the nurses gave me two sealed envelopes: One to be sent to USCIS for AOS, and one for my personal copy.

Highly recommnend this clinic and Dr Benson for anyone around the Akron area.

Comments: This is the center doing medical exams for immigration. It's very easy to talk to them that we want only vaccination transcribe and the cheapest one I found around boston area. The cost for each additional vaccination is around $20-30 each.

They now charge $90

My kid and i vaccination paperwork: $180 , Chicken pox 2nd shot for kid: $20 = $200

The cheapest and closest we found that only did trancriptions, they even provided the forms with signatures, very helpful and communicative.

Had to go with 1 u.s passport style pic for each and show all vaccinations (i didnt have our DS-3025, so i provided all proof of vaccinations we had and they accepted them even when some of them were in spanish; im colombian, they knew the language)

Dr. Anthony's place is great. We walked in and told them about our situation and the lady at the front desk knew exactly what we needed. My wife needed 1 more MMR shot, so he sent us to Fort Green Center in Brooklyn to get the shot for a couple of bucks. We came back afterwards and they gave us the form in a sealed envelope. He charged us $60 for the service.

My husband already had all of his vaccines and other requirements but they did tell him when he first got there that it would be cheaper to go to his regular doctor, get needed vaccines and just come back with the paperwork. But I was also told that by several different doctor's offices that wanted going to charge between $175 and $ 250 for the basic exam. Nice people, just a really long wait

Price: $75 for adult, $50 for child (the best cost/value ratio in the whole Metroplex, after a very extensive research!)

The price includes filling out forms I-693, physical exam, blood test (for syphilis / RPR) and also the skin TB test (TST/Mantoux).

The clinic was willing to accept external lab results (I wanted to have iron-clad proof of immunity for Varicella (chickenpox) instead of just relying on "verbal history") and for QuantiFERON TB Gold - instead of TST, to which we would test positive (coming from a BCG-vaccinating country). We didn't want the extra radiation knowing that the skin test will end up being false positive, so searched on-line and with various labs/health departments.

There are private labs who offer the QuantiFERON tuberculosis test for about $150 (after coupons), but we also checked with local / county / adjacent cities' health departments who run disease / TB prevention programs and found one who accepted even non-county residents for $60 for the QuantiFERON test. We ended up getting to one of the Theranos locations for $42.18 per person, and also added a Varicella test for $8.77 to eliminate any subjectivity. Got the results in 3 days via Theranos' app, brought them back to Prestige Primary Care - who were happy to accept and attach them to the form.

Overall, highly recommend the clinic. After much research (believe me, tens of hours on the net and on the phone, registered as a new user just to share the information with the community which helped me learn) this was hands down the most cost-effective solution I could find in the whole Dallas - Fort Worth area - on top of excellent, professional service. Hope this helps others, and Dr. Aziz will get the volume of clients to keep the costs down. We were quoted up to and above $1,000 for the family, before any tests or vaccinations (!) - so I voted with my money for Prestige.

Last thing - check with your employer whether they will cover the costs (check-ups and vaccinations for you and the family) - I tried my luck and was pleasantly surprised!
