I know that there are tons of threads regarding this same question, but some are outdated or answers are round-about and I just need a straight answer/the facts so I don't mess this up.

My husband came here on a K-1 visa. His medical exam was completed 02/05/2009 (this was his 2nd medical that he had to pay for because he was in AP for 6 months - grrr) Anyway, What portion of the I-693 does he need to have completed? What do I need to receive from a civil surgeon? We have a copy of his DS-2053 Vaccination Documentation Worksheet. I am trying to figure out exactly what I need to do regarding I-693 / his medical.

- 06/30/2009: Filed for AOS/EAD/AP

- 07/02/2009: AOS packet received

- 07/08/2009: Check cashed

- 07/10/2009: Received all 3 NOA1s

- 07/14/2009: Received biometrics appt.

Usually it's just the vaccanation sheet you send in. (they should have the medical he did for the visa)

If they want more (medical lost/out of date) they will RFE you for it.

My Advice is usually based on "Worst Case Scenario" and what is written in the rules/laws/instructions. That is the way I roll... -Protect your Status - file before your I-94 expires.

Some people here have talked about having a Civil Surgeon transcribe your vaccination sheet to the I-693. Is this necessary or can we just send in a copy of the vaccination sheet that was signed by the Civil Surgeon in Turkey who performed the medical exam?

I wouldn't see why you couldn't use the from that was signed in Turkey.

You didn't need to get any more shots, when you came to the US, right?

You can try, but the directions say to send in the I-693. Some VJers have been approved with just sending in the CS-3025. The decision to take a chance on an RFE is a personal choice.

If you send the vaccination form from Turkey, be sure that it's fully in English or that it's fully translated into English, with the standard "translator's certification" available on VJ.

06-04-2007 = TSC stamps postal return-receipt for I-129f.

06-11-2007 = NOA1 date (unknown to me).

07-20-2007 = Phoned Immigration Officer; got WAC#; where's NOA1?

09-25-2007 = Touch (first-ever).

09-28-2007 = NOA1, 23 days after their 45-day promise to send it (grrrr).

10-20 & 11-14-2007 = Phoned ImmOffs; "still pending."

12-11-2007 = 180 days; file is "between workstations, may be early Jan."; touches 12/11 & 12/12.

12-18-2007 = Call; file is with Division 9 ofcr. (bckgrnd check); e-prompt to shake it; touch.

12-19-2007 = NOA2 by e-mail & web, dated 12-18-07 (187 days; 201 per VJ); in mail 12/24/07.

01-09-2008 = File from USCIS to NVC, 1-4-08; NVC creates file, 1/15/08; to consulate 1/16/08.

01-23-2008 = Consulate gets file; outdated Packet 4 mailed to fiancee 1/27/08; rec'd 3/3/08.

04-29-2008 = Fiancee's 4-min. consular interview, 8:30 a.m.; much evidence brought but not allowed to be presented (consul: "More proof! Second interview! Bring your fiance!").

05-05-2008 = Infuriating $12 call to non-English-speaking consulate appointment-setter.

05-06-2008 = Better $12 call to English-speaker; "joint" interview date 6/30/08 (my selection).

06-30-2008 = Stokes Interrogations w/Ecuadorian (not USC); "wait 2 weeks; we'll mail her."

07-2008 = Daily calls to DOS: "currently processing"; 8/05 = Phoned consulate, got Section Chief; wrote him.

08-07-08 = E-mail from consulate, promising to issue visa "as soon as we get her passport" (on 8/12, per DHL).

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).

THERE ARE PROBABLY OVER 100 THREADS DEALING WITH THIS VERY QUESTION, AND THEY ARE VERY RECENT WITHIN THE PAST MONTH. :unsure:

Great - if you could post a link to one of those threads it would be appreciated. I read through several and didn't seem to find a clear answer.

Since the visa medical is less than a year old, its good. You just need to send in the vaccination supplement. For this, you can take the DS3025 to a Civil Surgeon along with a I693 (complete the Part 1 bio info). Civil Surgeon will transcribe the vaccination info to Part 2, section 5 and fill out part 5 (civil surgeon certification). They will give this form back to you in a sealed envelope to include in your AOS filing package.

You will definitely have to get the I693 vaccination supplement if he needed to get addition vaccines after entering the US to meet the AOS requirments. If not, you can send in a copy of the DS3025. Some have done this with no problem, but you could get an RFE for the I693 supplement.

Since the visa medical is less than a year old, its good. You just need to send in the vaccination supplement. For this, you can take the DS3025 to a Civil Surgeon along with a I693 (complete the Part 1 bio info). Civil Surgeon will transcribe the vaccination info to Part 2, section 5 and fill out part 5 (civil surgeon certification). I just noticed that under the "Results" section on the DS-2053 "Vaccine history is incomplete" is checked, and then below it "Applicant may be eligible for blanket waivers because vaccinations not medically appropriate (as indicated above)". What does this mean?

It looks like his MMR and Varicella vaccines are complete. He received two Td or Tdap vaccines (7/16/08 first medical and 2/05/09 second medical due to AP), but "Insufficient Time Interval is checked". All of the other vaccines are checked "Not age appropriate". Where can I find info about what vaccines he needs for AOS? I don't want to get screwed. He has already had to pay for two medicals in Turkey!

JuliaFerno if you want to be safe it only costs $20 (or there abouts) to get all of the information transcribed into the I-693, Dr Arnolds office will even give you a list of all the vaccinations that your man will have to have it might be worth calling the office purely for that reason
