Adjustment of Status

The instructions for adjustment of status from the Consulate are not detailed. I've reviewed the forms on USCIS website. I just have a couple of questions:

When you fill I-485 do you write your married name for your last name or do you keep your maiden name so it matches the name shown on your passport and visa you entered the country with?

The instructions from the Consulate say you mail the package to the service center having jurisdiction over your residence but USCIS says mail it to a central address Chicago. Which is the correct one?

You write your married name on the form....

Use married name on the forms, if you dont and later decide to change name on green-card you will be filing an I-90 and paying a FEE.

There are lines on I-485, and G-325A for maiden name. "Name on I-94" and "Other names used"

Use whichever name you wish to be legally known as now. If you prefer to keep your maiden name, use that on the forms. If you wish to be known by your married name, use that.
