I am filling out the I-129F and I'm not sure how to fill out the information for the Petitioner's Birth Certificate. I checked the box for "Birth in the U.S." and it asks for "..number of certificate, date, and place it was issued." It was issued by the Los Angeles County Registrar, and the certificate number I have, but the date confuses me. I have a birth date, a date accepted for registration, and a date it was stamped with the seal. They're all different dates. Which one do I use? Thanks.

I suppose it should be the date with the stamp and seal as this is the date that the document became official. Besides, the petitioner's brithdate was already in Section A.4

Just check birth in the US and provide the birth certificate. The second part of your question doesn't apply to you. It applies to those who gained their citizenship in a way that required a certificate of citizenship, on which there is a number, date and place issued.

Yes :) I did not put a certificate number or anything there, just checked birth in US & provided copy of birth certificate & it was approved just fine :)

You're not leaving anything blank. It's just that the form is not well organized.

You have to check one box for question 10. If you check "Birth in the US" then you're done with the question. If you check "Naturalization" then you're supposed to provide the certificate of naturalization number. If you check "Parents" then you're supposed to answer the question "Have you obtained a certificate of citizenship in your name?" by checking "Yes" or "No". If you check "Yes" then you're supposed to provide certificate number, date, and place of issue.

The above probably seemed perfectly obvious to the bureaucrat who designed the form. :wacko:

Put N/A when a question is not applicable. "Doesn't apply" = "N/A" = "Not Applicable".
