Hi all,

I have seen conflicting instructions for how to complete the I-693 form for the AOS. The directions state that because my husband had a medical review as part of his K-1 application, he only needs to fill out part 1 and have the civil surgeon fill out the vaccination part of part 2. VJ's guide says that I should ignore those instructions all together and the form is unnecessary. Does anyone know what I should do? Do we need to include that form?

Do as the directions say. If you have problems with the civil surgeon in your area use Dr. Arnold.

Last I saw he charged $20.

Just go to the designated surgeon by the USCIS near your area and i'm sure they had a i-693a form for the vaccination. Explain to them that your husband came in K1 visa and just need the required vaccination for his AOS.

You need to include evidence of vaccination... Up until May 2008 the I-693 did not have vaccinations as a component part of the form but you supplied a supplement... This is now different as it is a recent change

I-693a hasn't been valid since May, 1 2008. They only use the I-693 now.

Whatever she/he is she/it seems the best way to go if you have problems in your area.

I did see where a IO questioned a couple why they used a civil surgeon from another state and they explained why and didn't have any problems.

Wish I'd known about her when we did ours. Cost us a lot as we had to go to another island to get it done as there aren't any on our rock!!

dr arnold is just good if you need to get the ds3020(?) (vacination doc from your k1 medical in your homecountry) transferred to the i-693a. that works over fax or mail.

she is not going to complete the i-693 which is the complete medical examination, without seeing you.

as someone wrote, they must have changed something with the medical requirements for aos in may, i dont know what they changed since i already have my gc and dont have to worry about that anymore. do what the instructions for the aos package (I-485) tell you.

What's the point of transferring onto I-693a if that form is no longer valid?

I-693a is no longer valid. The CS will complete parts 2 and 5 after you complete part 1 of I-693. In the past, many people filed with just the DS-3025 (did not get it transferred to an I-693a) and were approved. The new I-693 form went into effect on May 1, 2008. You may be able to get by with just the DS-3025, but the change is too new to have sufficient evidence one way or the other. Our CS filled in the I-693 part 2 and 5 for $40.

Just go ahead and file it. Don't worry about it.if you had medical examination and vaccination supplemet done as part of the k1 visa process within 1 year , you don't need to send that form .just let the service know about that on the cover letter for your AOS.i sent mine without that form and i knew people who did the samething, and it's not a problem because they have your medical report in the packet you handed at the point of entry.

i asked the immigration services and they told me i didn't have to send the form .you can call them anyways and make sure yourself .1-800-375-5283

well thats why i wrote that they changed something about it but i dont know what and therefore follow what the aos instructions say!
