What do I need to bring with me for my medical Exam?

So I am quite a ways away from my medical exam but want to start preparing for it. I was wondering for those of you who have done their medical what do I have to take to my medical exam besides my appointment letter and all that paper work?

 

I know I need my immunization records is something I need to bring with me. Besides that is there anything else I need to bring with me? I am in my mid 20's have clean bill of health. Never had any surgeries, never was on medication for anything (besides cough, cold and other normal things). So what should I be requesting from my doctor besides my immunization records.

Timeline

You will also need 4 passport style photos and a note from your doctor stating that you are in good health. I'm not sure about the last one, prior to my medical I looked up the info on the clinic's website and it said to bring that note from the doctor as well. The best thing would be to call the clinic/doctor's office and ask them what to bring.

Also note that some clinics require 5 photographs instead of 4 (you can look it up on NVCs website) and you will also need your passport, forgot to mention that.

So no need to request every single note  the doctors office has on me?

 

I did call every single doctor authorized to do the medical in Canada and for some reason no one answered the phone and all of them directed me to the website if i wanted to make an appointment.

Well went to the state department website and it said something to the lines of "your medical history will be reviewed", was not sure if it means the doctor will just ask me about my medical history or if i should take documents.

Jan 13th 2016 - Mailed our reply to the 221(g) to the US Embassy, received and CEAC updated the next morning


Jan 20th 2016 - Embassy require more in-depth info on asset for i-864


Feb 1st 2016 - Sent more in-depth info on assets as requested. Received the next morning


Feb 16th 2016 - Visa has been issued :dancing: :dancing: :dancing: :dancing: :dancing:



In the US


April 5th 2016 - POE Newark. No questions asked.


April 14th 2016 - SSN received


May 10th 2016 - First day at my new job :dancing:


May 27th 2016 - Green Card received


June 7th 2016 - Got my Texas driver's license
