Okay I just had interview few days back. You def need notarized english translation of ur nikah nama, also must send original urdu nikah nama. To be on perfect spot, take all three types. But original and notarized english translation is must. YOU MUST MUST obtain police clerance certificate. I Didn't and it has posed delays and problem. GOOD LUCK

Where do you live? Go to near Nadra office and get a copy from there. Embassy do accept Nadra, take original documents with you. If you have children and are in application then yes take all documents with you. Sometime they do not ask but take everything with you. Good luck

I don't think embassy accepts bay form. They will ask for original birth certificate. It's better to have these ready rather than sending afterwards to avoid months of waiting.
