It looks like the req's are age specific so ages 19-49 are req'd to have Tdap, MMR, Varicella, and flu during flu season (flu season ends March 31st) i'm not sure about regional req's... I didn't see anything about that anywhere... but anyways I thought this might be helpful for you!
 
And I'm not sure how much the req's vary from country to country because the req's are for the US... For example, if I was traveling to Thailand and needed the vaccine for yellow fever (just making this example up) that is a req to travel to Thailand. If I were coming to the US FROM Thailand I would imagine that yellow fever would be something they rule out during your medical exam and not necessarily require you to have a vaccine for it seeing as you will be in the US and that is not a concern here... Does that make sense at all?

Vaccines can be done by anyone (as some usually are done as a baby) but the last approval that says this person has all vaccinations completed for immigration has to be an approved clinic from embassy.

there are cases that gets approved that sent after 1-2 months of relationship. Believe me their reason was not that.. the embassy just didn't believe their relationship was bonafide.
Yes vaccination paper is important for AOS. Definitely get a copy from the doctor when you finished at the clinic.

these approvals are mosty the first week of august I think. Maybe they updated after I finished the first scan.. I wish I could see further in august but so far nothing.
