K1 to AOS. I got an RFE few days after the interview for I-693 medical. I did the complete medical for k1 and applicant voluntarily completed vaccinations box was ticked. IO also confirmed they had my medical from K1 at the field office interview.

Now polio was waived in my k1 medical, so I believe that's what they want from me now.. to get the polio vaccine. But is the RFE also asking me to do the entire medical again also??

Please wise folks, lend me your understanding. Also is there a USCIS email or something I could ask them directly to clarify what they want and have it in writing? I'm so close to the finish line, I'm stressing. I don't want to mess up now.

You should only need to get the Polio Vaccine and then have the Civil Surgeon complete the Vaccine Assessment part of the I-693 (if you can find one that will do just the assessment). There is a box on page 1 of the form #4.A. you should check indicating you are eligible for the Vaccine Assessment Only because you completed your Immigration Medical Exam overseas. Make sure you bring proof of the Polio Vaccine being administered (we got ours done at a CVS) AND bring a copy of the DS-3025 Vaccine Worksheet from the medical exam overseas. If you can't find a Civil Surgeon that will do just the assessment (all too common unfortunately) you will have to do the whole exam. If this is the case, still bring the copy of the DS-3025 and the proof of the polio vaccination.

I thought so too. I contacted Dr. Harris office to setup the appointment at CMC. I haven't communicated with him directly but the lady staff at his office told me to email my RFE and she is now concluding I need to do the whole medical. So that got me all confused again.

Oh Lord.... Our friend in Maryland got the same exact RFE and they did just the Vaccine Assessment, sent it into the USCIS field office as instructed and were approved 7 days later.

I just checked my wife's report and Saint Lukes did this to her too. Polio is marked "A".

@2022 Unfortunately, most if not all Civil Surgeons will charge for a full medical to just transcribe the DS-3025 to the I-693, and charge extra for any vaccine or other services needed on top of the full medical. In our case, we ended up paying 545. You can read our experience at

It's $390 for the full medical where I'm going. Thanks to Edward, I know this doctor didn't rip him off and charged $50 for just doing the necessary paperwork. I guess since the reviews appear like USCIS likes this particular doctor, I'll do what he says.

I just wanted to know from the USCIS English RFE (let's be real, it confuses so many of us), what is it that USCIS wants? Just the polio from me or the general medical??

You have the USCIS English RFE from your original post.

Just give it to the Civil Surgeon.

The Civil Surgeon determines what you need and charges accordingly.

If you have the time to shop around for the lowest fee Civil Surgeon, then that is your choice.

In our case, we just wanted the fastest turnaround to meet our schedule.

You were able to get just the vaccine section updated and not have to pay the whole amount?

My child and I both got RFE for new I-693s after our interview in December. In our case ours had expired, dated just two months before the non-expiring forms came online. That initial non-expiring form has now been superceded by the form requiring polio vaccines. We weren't aware of the changes to the form until we got to our medical. Luckily I have fully digitized records for my child's polio. While mine aren't, I was able to confirm that I'd been fully vaccinated. The Dr mentioned it had to be the whole polio course.

We were also told my child needed the Hep A which they didn't last time (don't know why), and we had to get flu shots because we were having the medical done in winter. In August, it doesn't matter, how arbitrary. We were missing one of the receipts for child's three part Hep B so that had to be done again. On the plus side, I had aged out of some lab tests, but having to rerun required labs, blood tests to confirm immunity, get additional vaccines and pay for new medicals ended up costing over $1000. Very frustrating that we were just outside the expiration window. The long processing wait is one thing, shifting goalposts while waiting is another.

Hey, I ended up doing the whole medical. The doctor I went to accepted my previous K1 vaccinations and I got the polio and flu vaccines for free from CVS pharmacy (Cigna insurance).

But the civil surgeon thought my RFE wants the physical and blood work medical and I did it just to be safe. It came to $390 for me.

So annoying. Sorry you and your kiddo have to go through this too.

So sorry for you too, it's a lot of stress, especially this close to the finishing line. They'd already sent us an RFE previous for updated documents, I'm not sure why they waited until to interview to tell us our medicals had expired 3 months ago.

The irony is, because we used the same CS, we gave him our expired copies and he just sat and copied everything from the old form into the new one. The only thing he needed to check was the vaccine receipts we'd gotten since the last form was done and Polio since that was new, we had no other medical changes. And the only labs either of us needed this time was TB, so even less work. Ho hum. It will be worth it.
