My wife's family wants to get visas to come visit us (from Peru). Her parents I am not worried about at all as they both have career jobs, have a house, bunch of property, etc. However her brother only works for his dad and lives at their house. He had brain surgery last year to remove a tumor and has occasional seizures when he gets lax on his medication. My wife's mom does not want to leave him alone as she has to be there for him although he is generally healthy. Does this factor in at all in the application process? Obviously he doesn't have a case for "strong ties to the country". My wife got rejected for visitor visas twice before our K1, so they are worried about it. Not sure if any other visas would qualify if they wanted to get him looked at by a doctor here (want to see if there is any kind of other medication he could take). But their main goal is just to visit us so don't want to come across as untruthful to the embassy. Any advice is appreciated.

The entire family applying for visas might be an issue… 

you don’t know if you should or shouldn’t worry about any family member because the totality of circumstances is examined. 
some embassies won’t even speak to the applicants they’ve decided if they’re going to issue the visa on the strength of the ds160. 
you might be surprised and discover brother has no issue obtaining his visa while parents have a harder time because they have more of a path to becoming  LPRs

To apply for medical B visa, the applicant would have to show documentation from a doctor on why they need treatment in the US, or that treatment is not available at their home country.  If your BIL already had brain surgery in Peru, it would be questionable to claim that he cannot get adequate treatment there.  I suggest just pursuing a regular tourist B visa in line with their actual purpose of travel.

Especially since any anti-seizure meds would be available in both countries.

There is a specific variant of the B2 for Medical purpose.

 

As for all Visitor Visa's they apply and find out, take it from there.

 

From prior posts the best I can advise you is to stay out of it, nothing you can do and there is only downside.
