Certainly no paternity affidavit, and I expect all the mother has to do is state the father's name.  I do not know this is true for every state, as the state is free to have their own regulations in this regard.  The question can surely be answered by the mother asking the hospital where the initial birth certificate will be prepared.  There is NOTHING the foreign father can do to force or control this.

In most cases the mother can add anyone she wants as the father on the birth certificate whether they are the father or not without the actual father's permission!

DNA/paternity testing is not routinely done unless there is a specific court order or legal reason.

Yes, and then the right or wrong father can take legal action to get the birth certificate corrected.
