Hello!!

Just want to consult before booking our flight.

Got my greencard and Im planning to visit home for urgent reasons (PI)

the thing is, I haven’t had to chance to update my 🇵🇭 passport due to lack of appointments in DC. (No available passport schedule as of the moment and been trying to schedule it for quite sometime now but I always end up with no spot available for the month)

My question is, Can I travel abroad using my green card (with my new last name) and passport (with my old last name)??

Yes you can, but make sure to bring your marriage certificate as proof of your married name.

Yes you can.  Book the flight in the passport name and bring a copy of your marriage certificate in case CBP asks about it when you return.  Many do this without any issues.
