I contacted NVC to let them know I was changing process to AOS and I am waiting for my green card to be granted, I just had my biometrics earlier this week and I am waiting for my EAD and AP.

An attorney told me to write:

1. Yes

2. Lima, Peru (in my case)

3. In decision I put pending, since I never replied to the RFE I received. 

4. In date of decision I put N/A

 

I dont know if in your case it would be the same with you didnt go through the whole process, or if you would write DQ and the date they notified you about it.

Your wife should answer "Yes" to that question.  Since her case was already DQ'ed, that means she already submitted her DS-260 visa application form.

And what if the case hasn't been DQ'ed yet, meaning you haven't sent the affidavit of support and DS-260 and are just seeking to switch to AOS?
