Ok thanks, in Part 3 questions 1 & 2 do I put the La Paz Bolivia Embassy where they gave her the K1 visa in her passport or do I use a service center like this old post from 2016 I looked up on VJ...I’ll paste a view of that old answer below. I think I use the La Paz Embassy but I’m not sure.

I am really lost, I have tried looking around online but I still can't seem to find a certain answer. What am I supposed to write? How do I know in the first place where I filed? I can't remember... My case # starts with MSC, does that mean I should put National Benefits Center, MO as my answer for 3.2? And as for 3.1 what do I put? 😕

Complete Item Numbers 3.a. and 3.a1. if you entered the United States with an immigrant visa.

I came in on a K-1 visa. I assume I need to file 3.a and 3.a1 then? My POE was JFK so I write New York??  K1 is not an immigrant visa, you did AOS in the US so you will skip 3a adn 3a1 since there are not applicable to you.

It's asking about I-485 (in your wife's case). Thus it can be interpreted as either National Benefits Center or your local USCIS Field Office.

Ok thank you very much! I noticed in part 3 in questions 1 and 2 it says immigrant visa (k1 is nonimmigrant) or adjustment of status. 
 

I did the greencard interview at the USCIS field office  in Hialeah, FL and the Approved Notice of AOS I 485 has NBC Lees Summit, MO

original marriage certificate 

DL

SS card

Passport Bio page

NOA I 797 I-485 approval for AOS

USCIS Section 216 Notice with officers hand written notes on it.


Mailing to the following:

USCIS

P.O. Box 21262

Phoenix, AZ 85036

Thanks, I should have looked at sooner I don’t have an excuse just being stupid I guess.
 

I mailed it back Saturday 10/5  USPS  via certified mail BUT I’m still waiting on it to arrive in Arizona. I’m so stressed with the USPS they give me the canned statement ‘it’s in transit’......it’s been a dam week and it still hasn’t arrived and Im worried my wife’s greencard is in that package lost and how would I explain this cr@p to USCIS that I sent back a greencard to get a new card with correct dates and the USPS lost it!?!?!?!? 

 

Im so stressed with the this !
