I'm a DV Lottery Winner of this year from Egypt, i have a brother who live in the US. which form should he file to sponsor me I-864 or I-134 and where to file ? 

I-864.   Follow the directions in the instructions.

April 30, 2018  Mary moves from the Philippines to Mexico, Husband has MX Permanent Residency

June 13, 2018 Mary receives Mexican Residency Card

June 15, 2018  I-130 DCF Appointment in Juarez  -  June 18, 2018  Approval E-Mail

August 2, 2018 Case Complete At Consulate

September 25, 2018 Interview in CDJ and Approved!

October 7, 2018 In the USA

October 27, 2018 Green Card received 

October 29, 2018 Applied for Social Security Card - November 5, 2018 Social Security Card received

November 6th, 2018 State ID Card Received, Applied for Global Entry - Feb 8,2019 Approved.

July 14, 2020 Removal of Conditions submitted by mail  July 12, 2021 Biometrics Completed

August 6, 2021 N-400 submitted by mail

September 7, 2021 I-751 Interview, Sept 8 Approved and Card Being Produced

October 21, 2021 N-400 Biometrics Completed  

November 30,2021  Interview, Approval and Oath

December 10, 2021 US Passport Issued

August 12, 2022 PHL Dual Nationality Re-established & Passport Approved 

April 6,2023 Legally Separated - Oh well

(Unusually, @Paul & Maryis wrong on this one). I134 is the correct affidavit of support for DV selectees. 
(See for example the very last FAQ in the DV instructions https://travel.state.gov/content/dam/visas/Diversity-Visa/DV-Instructions-Translations/DV-2018-Instructions-Translations/DV-2018 Instructions English.pdf ) 

 

I864 is for family and employment based applicants where the petitions require a sponsor. DV is self sponsoring and an affidavit of support is not required, but may be requested in cases where public charge considerations can’t be overcome. , Per 134 instructions, “Form I-134 may be used in any case in which the foreign national is inadmissible on public charge grounds, but in which he or she is not required to have Form I-864 filed on his or her behalf”.

It depends on what kind of visa you are applying for and where your interview will be held. in my case as a DV Winner and my interview location is cairo i don't need this form but i was thinking about taking it with me as an extra support.

This partly depends where you interview. Sri Lanka requires an i134. A number of other embassies prefer it. 
 

Your payslips are not really relevant unless you are employed in the US and doing adjustment of status. By insurance you mean lining up US health insurance? That’s always a good idea. For most embassies, being able to show you are willing and able to support yourself is enough, but the lower level the job the more beneficial an i134 is. Savings, assets, education all count. When I got my DV visa it was quite rare for people to be asked for an i134 but it is becoming less rare, though my understanding is still that most people don’t need one.

I would always say, if you are able to get one then do. Having one can never harm your case.

I have one full time and a freelance job, both in Zone 4. I also have a master's degree. I just wonder with my qualifications plus perhaps 20k+ USD of savings, how likely is the CO will ask for a i134?  My health insurance is only valid in Hong Kong I believe, but I just want to prove that this is how much I'm able/ willing to spend.

You probably won’t need an i134, but it’s up to the interviewing officer’s discretion whether or not to ask for one.

 

Your Hong Kong health insurance won’t be relevant.
