Hi! I had an MMR shot but not from St. Lukes Extension Clinic in Manila. 
 

I brought a document signed by my local doctor and the doctor at SLEC acknowledged it. 
 

I noticed that on my DS-3025 there was no check mark on the MMR and only a date for when I had my MMR shot from my local doctor. There was no Blanket Waiver or code. 

 

Does this mean during AOS, I need to fill out an I-693 and have a USCIS accredited Civil Surgeon make an official form and say my MMR shot is valid?

What box is checked on the ds3025 bottom left?  Complete?

You are fine with the DS-3025 as is.

 

With the I-485 USCIS does require the DS-3025 be transcripted to the I-693.   You do not complete the I-693, the Civil Surgeon does.

You are failing to consider the k visa exception.   If the ds3025 is marked COMPLETE a k visa holder is NOT required to have it transcribed to the I693.

Not according to the I-485 instructions, which is the governing form.    The instructions clearly state that the I-693 is required, then it states that it can be partial or completely "depending" and to follow the I-693 instructions for what portion needs to be completed .. which in the case of the K-1 only the vaccination portion is required.

 

There is not an exception noted for having a completed DS-3025.   Do read # 10 of the required evidence to be submitted, it lists the I-693 ... with no mention of any "exception"

 

 

What Evidence Must You Submit with Form I-485?

 

" 10. Report of Medical Examination and Vaccination Record (Form I-693) Form I-485 applicants for adjustment of status are required to have a medical examination to show that they are free from health conditions that would make them inadmissible. This does not apply to registry applicants and individuals born under diplomatic immunity in the United States. If you are filing Form I-485 under the nonimmigrant fiancé(e), asylee, or refugee category, see the Form I-693, Report of Medical Examination and Vaccination Record, Instructions for more information on whether you need to submit the full Form I-693 or only certain parts because you already had a medical examination overseas. "

 

USCIS does offer the option to submit the I-693 with the I-485 or later during the interview, but does not offer an option to NOT provide the I-693 completed by a Civil Surgeon.

 

"You are NOT required to submit Form I-693 at the time you file your adjustment application, but may do so if you wish. Because of the time-limited validity of Form I-693, you may choose to submit your Form I-693 after you file your Form I-485. You may also submit Form I-693 in person at an interview in a USCIS field office, if an interview is required. By waiting to submit Form I-693, you may avoid having to repeat the immigration medical examination."
