Ok, so I called up the closest Civil Surgeon in State College, faxed them my hubby and step son's vaccination records, as well as the actual form they need to fill out for us, and I get a call back saying my husband (who is 24) needs a Hep B shot (that will cost $120 for the first) though he was told at the London medical for his K-3 he did not need the Hep B series. The CS also stated they charge $80 per person to fill out the vaccination form. *gasp* I am admittedly new at this part of the process but that seems like an awful lot of money to copy information from one sheet of paper to another.

What have others experiences been? Anyone in Pennsylvania have a CS they can suggest that would cost less?

$35 and only needed.

  • Mumps, Measles, Rubella (MMR)
  • Tetanus and diptheria toxoids
  • Varicella

You don't need to have the Civil Surgeon do the vaccinations, try your Family Doctor, Local clinic, or local health dept. The Civil Surgeon is only needed to transfer the vaccination records to the supplement.

Um, I hate to harp on this, but I strongly suggest completing the series of 3 Hep B shots required for protective vaccination. Like I've said before, there is a reason for a full vaccination regimen, not just to fulfill green card requirements. Hep B is serious, treat it accordingly. :angry:

I had the same experience in California. I went to the AOS interview without the supplemental form for the vaccination which delayed ma approval until I had it done. In ma case, the hospital charged $50 for the some other shots than Hep B and transferred the vaccination info onto the supplemental form and that was all. I would suggest , in the event that u are not able to get a civil surgeon whose charges are less than what u have now, u better go for the former so as to have ur AOS go smoothly. Good luck.

We are from Central New York, we are having the same problem about Civil Surgeons around the area, can I just have a required TD shot in a local clinic in our village and have the result and the DS-3025 sent to this civil surgeon and transfer to I-693A form from him? We'd sure to give a call before we send it in. Civil surgeons here are so insistent on having to have another tests for TB, HIV, syphilis -- a total of $267! Tests that I don't need all we need is a Tetanus-Diphtheria shot to complete my I-693A Vaccination Supplemental Report.

I just tried phoning them. I got the answering machine. I'm not sure if they are closed for the day or just busy. I will try again in a few minutes, or tomorrow if I can't get a hold of them. I'll let you know what they say.

I rang them earlier, and spoke to them, they were in shock that the CS in State College wanted to charge $80 per person. I sent the info off to them already with a check. Thanks so much for the info!

Hi, sorry to bump in like this, but the medical thing is exactly where my husband and I have gotten stuck. We're in FL and I had my medical done back home, no vaccination was mentioned then and now we dont know what to do or where to go regarding this. What's form I-693A? i downloaded I-693 and but i dont know which page/s we're supposed to get filled out and sent. Something else, my husband works in a hospital and i was wondering if there was a way we could do anything there. Anyway, i hope someone gives us some help and clarifies things a bit for us, it would be very appreciated. :) thanks

Saying this is a disservice to the people on this forum. It is NOT a requirement for green card or AOS. It is age specific, see other posts, or my thread at http://www.visajourney.com/forums/index.php?showtopic=69857

and as a dentist, it is not necessarily a general requirement for the public at large. YES, it IS for health care workers!
