Pretty sure you will be filling out DS-260. For my EB the DS-260 was unlocked once the fees were paid. Not sure but could be the same for the IR5

Following the link I gave you for the process at NVC, you will see that completing the DS-260 is right near the top once your petition is approved.

 

Do read the the link for NVC process and follow the steps as laid out on those webpages
