Anybody help on how to ensure flights together or about interview without medical yet

You won't know for 100% sure until the visa is issued.  However, generally it is a good sign as they could have told her that they denied her application. Also in term of processing, we can consider HCMC quite transparent and quite fast (compared to others like Pakistan, Morocco, etc.): if there is something wrong, they just let you know.  I have heard any case that had to go to long Administrative Processing.

 

After material(s) return (1:00PM-2:00PM except Wednesday on working days) , she will know the verbal confirmation result and turn in her passport (for verbally approved case).  In case, she lives in HCM or near by (like Dong Nai), she may get her visa within 2-3 business days.

Until they receive the medical results, the consulates won't approve or disapprove, so she is "pending" for now.  Did they keep her passport?  She probably got the blue sheet saying her case is pending until additional info is received.  Also, is your fiance in HCM City?  The exam results may be mailed to her, and she will need to deliver them to the consulate.  It can save some time and worrying if she can bring them to the consulate herself.

 

Regarding flights . . . When I tried to book flights for my wife and her daughter, and 6 months later for her son (with my wife escorting him), it was impossible for me to use online sites to book a round trip flight and one-way tickets on the same flight for my wife/kids on the return trip.  Online booking moves so quickly, I was never able to see the same flights/seats for round trip and one-way flights.  You can call the airlines and do it over the phone, but when I tried, it was $1000s more than the prices on the travel sites I always use (justfly and cheapoair).  I ended up getting one-way tickets to and from Vietnam.  It cost about $400 more (for 3 tickets), but at least we were on the same plane!  Even though you may choose your seats when buying tickets online and confirm them through online boarding 48 hours before the flight, the seats aren't truly assigned until you check your bags at the airport.  We we were together when I booked our flights, but separated on our Korean Air return trip (wife and daughter together, I was about 10 rows away).  The air crew was able to arrange some seat shifting so we were together once everyone was seated.

 

As others said, wait until her visa is approved before booking flights.  My wife was going to Vietnam in December already, so we booked her son's return flight before his visa was approved.  It arrived 1 day before they were due to leave!  That was a VERY stressful wait!!!

So she took the results from the medical exam today and they told her she would have her visa in less then 2 weeks but when you check the site it does show the 23rd of may as the last update but says 

"Your visa case is currently undergoing necessary administrative processing. This processing can take several weeks. Please follow any instructions provided by the Consular Officer at the time of your interview. If further information is needed, you will be contacted. If your visa application is approved, it will be processed and mailed/available within two business days"

What's the normal way for the beneficiary to schedule the medical ?   After the interview is scheduled (in which case the timeline is tight) or after the petition is filed? (Could be months). How long is the medical valid ?

You find the list of panel physicians authorized to do the exam for that consulate/embassy and call and make an appointment with them. You can only schedule an appointment once you have gotten an interview date. The medical exam report is valid for 6 months.

For Vietnam, you schedule your medical exam after receiving the interview invitation letter.  You'll call Cho Ray in HCM CIty, or one of the International Organization for Migration hospitals in Hanoi or HCM City.  I believe that information is included in the letter.  Those are the only three locations available for exams in Vietnam.  For Cho Ray, my wife was able to schedule an exam for herself and daughter within days of her call.  Same for her son when she scheduled for her son 6 months later.  They had their exam results about 4-5 days.  From our experience with Cho Ray, they were willing to schedule as quickly as possible to accommodate the interview date.

So today I checked ceac and it showed updated today and changed from administrative processing and went to ready and said to be there at the scheduled interview but there is no interview and she already had the interview.  They told her when she dropped off the medical results that she would have her visa and passport returned to her within 2 weeks.  Is this a standard change from administrative?  Is there actually another interview or is that just move out of administrative processing to issue the visa?

It will change again soon.  They only have a few options for the message.  It just means they have your documents and they are ready for the next step.  The key information is that it was "touched" May 28,  so they are working on it.  It may even go back to administrative processing as the visa is prepared for printing.
