Vaccinations Proof

Timeline

What kind of vaccinations proof you need to show to the doctor while your medical exam?  I visited my GP and she filled (by hand) so called special Passport of Immunization with history of my vaccinations and put her stamp in front of every entry.  Each entry is translated in English by default in that passport.

 

My card with my vaccines history is kept in health centre and she just re-wrote what was there.

 

Would it be enough? Or there is some special paper which needs to be filled in?

Take whatever you have in.  The panel doctor will determine what is needed based on your age.  Typically 3 to 4 shots.

July 14, 2020 Removal of Conditions submitted by mail  July 12, 2021 Biometrics Completed

August 6, 2021 N-400 submitted by mail

September 7, 2021 I-751 Interview, Sept 8 Approved and Card Being Produced

October 21, 2021 N-400 Biometrics Completed  

November 30,2021  Interview, Approval and Oath

December 10, 2021 US Passport Issued

August 12, 2022 PHL Dual Nationality Re-established & Passport Approved 

April 6,2023 Legally Separated - Oh well

All I can find is about if there are no records, or what vaccinations are needed. 

 

 I think for my age i have all vaccinations which are needed but was not sure how to present them since its not very common for our health centre to have such requests :)

I would ask the doctor while calling in to set up the appointment. They should know the list of things you should bring and also as a doctor from your own country, they should know what is available for you. In my case (Czech republic), I am taking my vaccination records plus the records from my general doctor.

The panel doctor makes the decisions to the vaccinations and does not have to rely on any evidence you submit from any other doctor., clinic or government.

 

Just go to the appointment and they will give you what you need for immigration.

 

My wife had no records and got the 3 standard shots plus a lollipop.

In my health clinic they have only old type brochure from soviet time with all my history since I was born. However, i have second one as well at home, so probably I take it with me. And this immunization passport too.

December 18, 2023:  Filed I-90 to renew Green Card

December 21, 2023:  Production of new Green Card ordered - will be seeing USCIS again every 10 years for renewal
