London never asks for extra evidence about the relationship. They aren't doubting it. This would be more about security clearance for you to enter the US. Read all of this for some insight into K1 screening https://2009-2017.state.gov/r/pa/prs/ps/2015/12/250747.htm

Isn't it common to get the 221g because of the medical results not being there yet?

That would typically get a "You are approved pending medical results" comment.

Oh no, I'm sad to hear that! It seems odd they didn't tell you why though😕 Remember it's not a denial, it's just sent back to the US, who already approved you, so try not to be too deflated! Hopefully they'll look at your case and see it as approvable, just as they did when they saw it the first time.

So is the whole case sent back to the USA? sorry they literally didn't tell me any information this morning!

No it's not. That happens when they think USCIS erred in approving the petition. Yours seems to be  just a wait (at the embassy) on security clearance.  They will not tell you what they are looking for when it involves security. Otherwise they would have said what you needed...proper birth certificate, affidavit of support, medical results...etc. You provided everything required or they would have said.

OP: did they keep your passport? 

Did your police report have an offense or a No Live Trace and if so did you provide a Subject Access Report from the police? 

We went into AP and got the 221g with no request for evidence and it took from Dec 14th to Jan 8th to go from Immigrant AP to Non Immigrant AP to Issued.

I dont know if it is me, but I dont think the uk us embassy website is very clear.
Could you please let me know where you book your interview appointment and pay your visa fees?
Also when exactly do you book the courrier to have your passeport back?
