AOS Interview - Form I-693, Report of Medical Examination and Vaccination

We have an upcoming AOS Interview and one of the requirements is says we need to bring is Form I-693, Report of Medical Examination and Vaccination.  It says this is optional if it has already been submitted.  We didn't submit this with our AOS packet as we were under the impression that the medical exam she had and the packet she received from that was this.  They took that from her at the POE and we never got it back.  Was that the Form I-693, Report of Medical Examination and Vaccination?  Or is this something else we need to do.  We are so confused.  Thanks.

Its the DS3025 which is same with I-693 if you already have a medical exam abroad no need to do it Again for AOS.. just bring the copy on the interview.

And make sure all the recommended vaccinations are completed

As long as the original medical prior to US entry was completed (including all required vaccinations) then you don't need to do a 163. That's for people who may have incomplete vaccinations, etc. I would take a copy of the 3025 you have to the interview just in case, but you wouldn't need to complete or submit the 163 so long as the 3025 is complete.

The original medical was completed prior to US entry.   Attached is her DS-3025 form. Is that good enough or does she need more vaccinations?  We aren't sure with all the waivers letters.  Thanks.

Theres a page two of that sheet.. and yes i saw that shes voluntary complete the required vaccines.. that is good to go.. im pinay so i know 😂😂😂

Looks like it's checked complete down there towards the bottom so I would say yes

There is a B on a lot of them. It means, "Insufficient time to complete series".  Is this something I have to worry about?  At the  bottom of the paper there is a check box that says the "K1 applicant  voluntarily competed  vaccination requirements". So is it completed and this is good enough or must I do something else?  We are confused now. Thanks.

I think you might only gonna need MMR ( measles, mumps, rubella) vaccination but to sign your ds3025 only you have to search about that more because mostly designated doctors in us don’t just sign the ds3025..they will probably ask you to do the whole new medical 

but in order to search for that doctor who will complete your ds3025, you need to check list of uscis approved physicians in your area and ask them if they only can give one shot and complete the vaccination form instead of whole new medical

How long you needed to wait to get that vaccination(mmr) did they mention in previous medical? 

And 

if that much time has passed then you have to get MMR vaccination 

if have not passed that much your physician told then let the immigration officer decide

NO! If it is marked complete it IS complete at the time of the exam. Nothing else needs to be done.
