Spouse visa applicants need a medical and vaccinations, just before the visa interview at the end of the process.  Spouses adjusting status from within the US, need a medical exam and vaccinations to include with their initial filing.

The foreign spouse needs a medical exam (it's not the same as a physical) regardless if applying for a spousal visa or AOS inside the US.

All intending immigrants applying for an immigrant visa must have an immigration medical examination by an approved panel physician.

For the UK, this would be at Knightsbridge.
