Mailing Extra Documents

I need to mail off my original birth certificate and a federal police report to the consulate in Sydney. I’ve done my interview already and those two were the ones missing. I’m just a bit confused about the consulates address:

 

U.S. Consulate General

Attention: IV (g) “SURNAME” [??]

Level 10, MLC Centre

19-29 Martin Place

Sydney, NSW 2000

 

Where I placed [??] is the part that confuses me. I understand my surname goes there but what’s the other parts?

Did you receive a 221(g) for these missing documents and therefor you are returning what they are asking?

Yes I did  So I'm assuming I put 221g inside the bracket. How about the IV part?

Also I was wondering if you knew whether for the Federal Police Certificate that fingerprints were required? I read in other places that code 33 complete disclosure was sufficient but wanted to be sure.

Just return what they are asking for. Make a copy of everything for your files in the event something gets misplaced. Include the original 221(g) in the packet and a copy for you.

Fingerprints are not required the 33 will be fine
