Is the K-1 valid for ENTRY up to 6 months after date of issue plus 90 days to marry after entry? Or is it valid for 6 months total and the 90 days to marry after entry must fall within the allotted 6 months?

It's six months from the medical. So if the medical was done in Nov or Dec then 6 months from that date.

Then the 90 days start on entry.

The above is correct.

To explain a little further, the K1 technically expires the moment you enter the US. Once you enter the US, the K1 is no longer valid as it was a one-way use. You still must then get married within 90 days. The 90 days have to do with your entry date, not the date of your medical or the date you received the visa in hand.

How far in advance is the medical before the interview?

If more than 6 months passes between the medical and adjustement of status, is another medical exam necessary?

It depends really. For me I had to book as soon as I could as the only place to have it booked up fast. So mine was a month before my interview.

The medical is good for 1 yr for AOS
