Ah, it does cover doctors abroad. I stand corrected there (albeit the instructions there only apply to AOS since those are civil surgeons, not panel physicians, and then defer immigrant visa cases to the DOS website). That said, the fees they charge for a medical for a visa do not differ. They are standardized fees by provider. See the PDF from the DOS for Santo Domingo:
