Marriage Date

Hi,

On the marriage application form issued by the state of FL, May 12 2012 is recorded as the day of our marriage. This is when we had the ceremony in church. On the top of it, May 15 2015 is entered as "official record" of the marriage with the receipt number.
For USCIS purposes, did I get married on the day of our marriage, or when I brought the form back to the govt?

Not sure how Maine works, but, the marriage date in the NY cert says it right there: "Date of Marriage".

The day the clerk registers your marriage in the county is the day you are recognized as lawfully married.

Under the 'Certificate of Marriage' section at the bottom, the box that says 'Date of Marriage' has the date you were married. I think it is #21.

The information at the top is about what clerk entered the information into the court system, and when. The date the clerk recorded your information is not the date you married. The date you married is the day you got married and the license was signed by the officiant and your 2 witnesses.
