Do not have DS3025?

I don't recall getting this form http://photos.state....2/iv/ds3025.pdf filled. I got sealed envelope when i had overseas medical and that was handed over at POE.

Since I don't have this form, is there way to get this in USA? Since my overseas medical was within 1 year, I don't have to fill out I-693 form.

The civil surgeon should have given you your own vaccination copy aside from what was in the sealed envelope. If you don't have it, then you will have to go see a civil surgeon here in the states to get the I693 done.

I have one copy that the clinic handed to me when I went there to get my medical result including my health certificate. The doctor also stamped and put vaccines labels on my vaccination book. They should've given you one copy.

I didn't get this form either. I had my medical in January. All my vaccines were completed then (they wouldn't approve my medical until all of them were done). I was given a CD with the x-rays that I was told to keep. And later I got the sealed envelope from the embassy. At POE I handed in the envelope. But I definitely never got this form and no one ever mentioned it. I sent off my AOS application a month ago, so we'll see what happens :/

Same thing happened with me, I only got an Xray CD, I sent nothing with my AOS packet.. hoping I won't get an RFE but will see what happens!

The DS-3025 will be included with all your other medical documents that were in the sealed envelope you turned in at your POE. If you had all the required vaccinations at the time of the K-1 medical and the DS-3025 was filled out correctly, then they will have that info in your file. You can point that out to them if asked, if you have an interview. If you get an RFE, you will probably just need to go ahead and see the civil surgeon for the vaccinations translation onto the I-693.

Those that have no sent off their AOS paperwork yet, add a note on your cover letter that you had all the required vaccinations and they are marked off on the DS-3025 that should already be in their file.
