The petitioner and beneficiary both need G-325a http://www.uscis.gov/portal/site/uscis/menuitem.5af9bb95919f35e66f614176543f6d1a/?vgnextoid=d7c84154d7b3d010VgnVCM10000048f3d6a1RCRD&vgnextchannel=db029c7755cb9010VgnVCM10000045f3d6a1RCRD

They must be signed and returned to the USCIS as originals and not copies. Both must also send signed letters of intent. There are example forms here on this site.

Thanks again! If i recall i saw one of the post from here saying that i need to send 4 for each of us?

That must have been a really old post. The form version that needed 4 copies has not been used in a very long time. One form for each of you. The guide lists everything that is required > http://www.visajourney.com/content/k1guide
