My wife and I are filling out form DS-230. Question #2 asks for other names/alias, and if married to give maiden name. My wife did not change her name after we got married, nor does she intend to. Should #2 be "none"?

It's very simple. Just repeat the same name as Q#1, just write first name and last name.

03/10/13: NoA2

04/29/13: NVC Received

05/10/13: Case No./IIN Generated/Received

06/26/13: Case complete

Does question 39 refers to the beneficary (applicant) or the US citiven(sponsor)?

Where should DS 230 be submited?

In question 30,i could not remember some of the months i relocated but could remember all the years. Would this be acceptable?

Month and year is needed. Guess. Was it spring? Summer? etc... I had to remember exact addresses for 18 moves plus month/year. You can do it. You'll be okay. good.gif

DS-230 and documents (aka IV package is sent to the address on the bar coded cover sheet.)

Q39... It asks for the name and address of the petitioner. The US spouse is the petitioner in this instance.

what did you all write for "course of study" for elemetary and middle school? "N/A", or the respective school level? Ex: [name of middle school] course of study: middle school? Another option could be "general"

I don't believe they are looking for elementary or middle school. But general studies should work.
