Can someone help me and tell me what to put here?

My uncle petitioned for us from USA.So will number 3 and 4 be from USA or my home country?Or one way or the other?

And IDK what to put for number 5 either A or B.

Thanks in Advance.Sorry for bad English.

BTW I already have my green card and it was a uscis error so Im applying to get that fixed.

N-400 Citizenship Timeline

07/05/2012 = Eligible to apply for Naturalization based on the 5-year rule (90-days rule applied)

01/11/2015 = Sent N-400 Packet (to Phoenix Lockbox Facility via USPS Certified Mail)

01/15/2015 = N-400 Packet Delivered

01/27/2015 = I-912 Approved

02/02/2015 = NOA1 Received (Priority Date 01/15/2015)

02/07/2015 = Biometrics Letter Received (Bio Set for 02/20/2015)

02/25/2015 = In Line for Interview

04/03/2015 = Interview Completed

--- = Oath Letter Received

04/15/2015 = Oath Ceremony!

06-04-2007 = TSC stamps postal return-receipt for I-129f.

06-11-2007 = NOA1 date (unknown to me).

07-20-2007 = Phoned Immigration Officer; got WAC#; where's NOA1?

09-25-2007 = Touch (first-ever).

09-28-2007 = NOA1, 23 days after their 45-day promise to send it (grrrr).

10-20 & 11-14-2007 = Phoned ImmOffs; "still pending."

12-11-2007 = 180 days; file is "between workstations, may be early Jan."; touches 12/11 & 12/12.

12-18-2007 = Call; file is with Division 9 ofcr. (bckgrnd check); e-prompt to shake it; touch.

12-19-2007 = NOA2 by e-mail & web, dated 12-18-07 (187 days; 201 per VJ); in mail 12/24/07.

01-09-2008 = File from USCIS to NVC, 1-4-08; NVC creates file, 1/15/08; to consulate 1/16/08.

01-23-2008 = Consulate gets file; outdated Packet 4 mailed to fiancee 1/27/08; rec'd 3/3/08.

04-29-2008 = Fiancee's 4-min. consular interview, 8:30 a.m.; much evidence brought but not allowed to be presented (consul: "More proof! Second interview! Bring your fiance!").

05-05-2008 = Infuriating $12 call to non-English-speaking consulate appointment-setter.

05-06-2008 = Better $12 call to English-speaker; "joint" interview date 6/30/08 (my selection).

06-30-2008 = Stokes Interrogations w/Ecuadorian (not USC); "wait 2 weeks; we'll mail her."

07-2008 = Daily calls to DOS: "currently processing"; 8/05 = Phoned consulate, got Section Chief; wrote him.

08-07-08 = E-mail from consulate, promising to issue visa "as soon as we get her passport" (on 8/12, per DHL).

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).

So you were in India, your uncle petitioned your mother/ father, you went for a immigrant visa interview in India, came over, got your greencard, but there was a problem with it? And you never actually adjusted status?

If so:

3: City in India where you lived when you attended the immigrant visa interview

4: City where you had your immigrant visa interview (Mumbai?)

5a: First place you lived in the USA

5b: where you entered the USA

(so for example if you flew New Delhi- Singapore- New York- Rochester (where you lived), 5a would be Rochester, 5b New York)

Me: Irish/ Swiss citizen, and now naturalised US citizen. Husband: USC; twin babies born Feb 08 in Ireland and a daughter in Feb 2010 in Arkansas who are all joint Irish/ USC. Did DCF (IR1) in 6 weeks via the Dublin, Ireland embassy and now living in Arkansas.

Because YOU applied for immigrant visa (ie interviewed in) - in New Delhi. Don't read any additional meaning to the questions asked - just answer what they ask you.

3/20/2009 - Biometrics notice received (no NOA1)

4/2/2009 - Biometrics

4/9/2009 - NOA1 date (first undelivered one is 3/5)

4/3/2009 - Touch?

5/6/2009 - ROC Approval - 65 days

6/22/2009 - CRIS Card production ordered email

7/7/2009 - GC arrived!

Naturalization Journey

3/03/2010 N400 sent to Arizona Lockbox

3/15/2010 Check cashed

3/17/2010 NOA1

3/18/2010 - Biometrics notice sent

3/26/2010 Early biometrics done at an ASC different from the one assigned to (Original BIO date was 4/15)

4/30/2010 Yellow letter received and info from USCIS mil line they are working on my interview letter (6/17 appt)

5/1/2010 Text and email interview letter sen

5/6/2010 Interview letter received - scheduled for 6/17/2010 at 10:05am

6/17/2010 Interview appointment - PASSED

number 3=my city where i lived when i went to interview... and number 4 us embassy New Delhi right?

number 5a) my current city in USA?i never moved...and 5b San Francisco cuz i landed there?
