If you go to your bank and explain that you need a statement for affidavit of support, they usually know what to print off for you. Explain that you need a statement showing the date account opened, present balance, and total amount deposited for the past year.

I didn’t find any country specific information on through the Embassy link:

Did you receive an interview letter explaining exactly what is needed for supporting evidence?

Just for clarification purposes. Mumbai is one of the few consulates (if not the only one) that sometimes instructs to have these forms notarized.

As a simple reminder, nothing needs to be notarized unless otherwise instructed by the Embassy. good.gif

You do not need bank account information or statements unless you are using assets to offset a shortcoming in your income. If you meet or beat the 125% requirement for your household size plus beneficiary, then you can just leave the assets section of the I-134 blank. Provide the I-134, most recent tax transcript, letter from employer and recent pay stubs to prove your current income. If you are self-employed, the most recent 1-3 years worth of tax transcripts with the I-134 will do. Nothing needs to be notarized.
