It says results:

Vaccine History Incomplete

She only got 3 vaccines" MMR & Tdap & Influenza

Do we still have to go see a CS and what are we required to get from them??

Yes you are still required to see a Civil Surgeon and wait for his instruction if you will still need to get a vaccination, i have the same case but my CS did not require me to get anymore vaccines. It is actually depends on the CS.

yes,you still need to go to the civil surgeon,today me and my husband have appointment for update my vaccinations,i have also same history like you,say Vaccine History Incomplete'' they gave me shot 3shot and my son have 5shot..we pay 10bucks for vaccine and 200us dollar for signature of civil surgeon and to update.. goodluck to you guys...

Yes, you still need to see the Civil Surgeon (CS). You need to have the CS transcribe your DS-3025 onto your I-693. Then you can send in your I-693 with your AOS packet.

Find a CS who knows the drill. We discovered that few CS's were familiar with the needs of a K-1er. You don't need a physical exam, and you don't need any more shots unless it's a follow-up Varicella shot. Chinook wasn't required to get the Varicella vaccination because she was over 25 years old.

We found a CS who completed the transcription in less than 20 minutes and charged us about $35.

She has VH there

Okay so at the time of her K1 she had:

- MMR

- tDap

- Flu

- VH

The rule is they must be up-to-date at the time of doing the medical, and based on that she is so she might be fine without seeing a CS. If I were you I not see a CS and just send a copy of her DS-3025. If it's not enough they will RFE her for it. So while processing the AOS I would research the CS's more so IF she gets an RFE (and based on your post I don't think she will) you'll know where to go. The other thing is they still (stupidy) lose medicals occasionally so you don't want to waste money on a CS when it's not sure you have to when they MIGHT make you do a new medical lately.
