I have a question about "residency last 5 years" on form g-325a. My fiance came over to the U.S. on a work and holiday visa from July 2010 to July 2011. Does he need to write this in his residency section, or is that still considered just a visit? And if he has to write it in there, will he need police records from here during the consulate step of the process? How would he get them?

If your fiance was working here list his time in the USA on the G-325A. USCIS looks for 5 years of past addresses so you can not have a gap for a year. Same goes when you are completing DS-156 for visa application down the road (excpet that form looks for 10 years of prior addresses).

You do not need to obtain police records from his time in USA, both the petitioner and the beneficiary's backgrounds are checked during the petition phase (as well as the NVC phase).

He needs to list it. He does not need police certificates from the USA

What if it is just a tourist visa? Does it still need to be listed? I stayed in the states for 11 months so do I put in the address I stayed in, or my address back home at the time?

Put the address where you stayed at in the US for those 11 months.
