We need help on the I-693(Medical form), we went by a Civil Surgeon's office to inquire about transcribing the vacinnation record from Nigeria and we were told my wife will have to do a new set of test, bloodwork and vaccines and they will be charging $300. I told the receptionist that we only wanted the vaccine records transcribed but she said the vaccine records from overseas don't count even when I explained that it was done 3 months ago. I'm at a lost right now.

Also, does anyone have information on a good Civil Surgeon in the NYC area?

China Vaccination record good in US, then at Civil Surgeon did TB and blood test and general exam. Dr incorporated out prior paperwork ( which came transcribed) and we were done. Look for new Civil Surgeon on line. USCIS - Designated Civil Surgeons - USCIS Home Page

On uscis site click find a doctor on right side under related links. It will show a map and click on NY lots of listings.
