form i-693 for interview for my aOs

me and my husband receive our interview date for my AOS this July 26, now we are confused because everything is complete except for this FORM 1-693 (medical examination and vaccination supplement sealed in enveloped??? what is this ?? because im done with my medical back in Philippines and when i arrived here at the US the immigration personel at the airport got my xray cd and was left for me is only the vaccination supplemental now there is also such civil surgeon ??? what will we do?? do we still need to file that form and bring in our interview date, and who are those they call civil surgeon? is my husband family doctor can check me and fill out the said form??? pleaseeee HELP US... tnxx

hi! i didn't go through k-1, but is there any "(unless already submitted)" thing beside the form i-693 on the checklist? if there are words like that, then you don't have to bring it or take another medical again because you've submitted it already.

I think you just need to do another Medical Exam here in the US. Just get it done! search online for local civil surgeon in your area.

The interview letters list what to bring, stating to bring them, unless already submitted. You had no RFE sent about missing vaccinations, so you are most likely fine. A copy of your DS-3025 vaccination sheet from your medical would have been included in the envelope you handed over at your POE, so they were already submitted. You do not need to go out and get a new medical, or the vaccinations. Just bring your copy of the DS-3025 with you to the interview, just in case.

search for a civil surgeon near your area.(you can use the uscis website for the search) try to call first and ask how much will it cost you. tell 'em that you already had your medical and you just need 'em to transcribe the vaccination form. I-693 is the form that they will give you.

It cost me $30 for that.

Don't worry abbout medical, i've been done my AOS and they did not asking about Medical here in US but i gave them my medical result from philippines and everything is fine, goodluck!!
