Hey gang!

There is some progress for Kittyfang. Medical is scheduled and I just wanted to double check that I have everything that is needed. I don't live close to the civil surgeon, so I had a few things done here, where I live, so that I wouldn't have to go back a second time.

I have T.B. test (negative, no X-ray needed) and Bloodwork (syphilis only since they do not require AIDS testing anymore).

I didn't have my shot record so I had to get some shots and titer here. I am above 27 years old, so (and if someone could confirm this, it would be awesome) all I did was get an MMR titer and tdap shot.

According to this (if you scroll down, you will see a chart) many shots are age sensitive so I don't think I will need anything else.

I decided not to get varicella shot because I had chicken pox when I was younger. I was told that declaring it is good enough for most doctor.

I am hoping that all I will need is the physical and that the doctor won't try to give me any shots I don't need. :whistle:

Well, I took my wife to a Civil Surgeon and they pretty much put her through the gauntlet. If you can get your shot records get them the shots will cost you about $1000.00 your medical insurance if you have any will not cover this. So you may wish to hold off on going to the Civil Surgeon until you have the shots completed at perhaps a local health department.

This is how I would do it if I could do it again.

Goto health department with forms from USCIS and let them see what shots you need and get them there. Then goto the Civil Surgeon with a copy of the shot records and all your other paperwork and I-693 form filled out by you preferably typed. He will do a physical on you that is very simple and then fill the paper work out for you. He will then give you one copy sealed in a folder DO NOT OPEN IT. You will send this copy to USCIS and then he will give you anpother copy for your records and three copies the Civil Surgeon will keep.

I went to the civil surgeon and got stuck paying a little over $1000.00 and to make it worse when they didn't have the last shot required I was sent to the health department to get it. And it was more then 50% cheaper.

My medical is on tuesday, and I have arranged for my vaccination records to be sent to me today from India. The list included Tdap, MMR, varicella , pdp and Hepatitis B. I would also get my TB test results here. Now, I am hoping the surgeon won't 'inject' anything, and it would just be a physical examination!

Usually they want the chicken pox on your medical records, and they need your shot records or you'll have to retake them all. (Painful on the body AND the wallet.) :o

Other than that you should be fine with just the MMR and Tdap. But PLEASE try to get your records.

As I've said, I don't have my shot record, so that is why I had an MMR titer. Also, if you see the link I posted to the USCIS document, many shots are age sensitive. I'm not sure why your civil surgeon charged you $1000. My phisical is $100, Tdap is $45 and MMR titer is $50 (ish). There is no other shots that are appropriate for my age, again, not according to Vaccination Requirements for Adjustment of Status for U.S. Permanent Residence: Technical Instructions for Civil Surgeons, December 14, 2009

The one shot that I am missing, as I've said, is varicella and it seems to run for about $45.

After calling a few places over the last month, I recieved mixed messages regarding varicella shot. I think it comes down to personal choices by the doctor.

I figure, if varicella shot is the one thing he really wants to give me, by all means he can do it.

Please make sure that your CS fill the form I-693 correctly and SIGN it.My CS (or her nurse) filled the vaccination part incorrectly and put some random dates which have nothing to my vaccination history. Check the copy of I-693 which they'd give you for your record....

I will! Thank you! You just reminded me that I need to call them and make sure they have the form or if they want me to bring it. I asked the nurse what I needed for the appointment and all she said what that I need a picture I.D... So I will have to call back and double check.

Kitty, just out of curiosity, the titre test was just for one type of vaccination?

most people get a titre test and it shows if they have an immunity to multiple things, mind you this can go for around 200 or so depending where you go. some insurances cover it some do not.

as for the chickien pox, verbal declaration by me was fine it all comes down to the civil surgeon you get though..some are knowledgeable others are just ripoff artists.

I only got the MMR (Measles, Mumps, Rubella) as well as Tdap (Tetanus, Diphtheria, Pertussis) because the other ones where not recommended for my age. For example, DTP/DTaP/DT is given to toddlers as well as Rotavirus, Hepatitis A, and Hib, Hepatitis B and Meningococcal are only given to teenagers, 18 years old and younger, etc.. I basically went down the list and took note of those I needed.

I don't have my records either. When I went to my CS he gave me all the shots and did the bloodwork and performed the TB test.

I'm 28 years old and the only shots he gave me were Tdap and MMR. He took my statement verbally that I had had chicken pox when I was four years old and transcribed that on the i-693.

He charged me $300 total. Everything was done then and there, 3 days later I came back for the blood and TB test result, and he signed and sealed the form in front me.

It was a fairly "painless" ( :rofl: ) experience.. :)

I had brought a one year old TB test I'd had done to my CS appt. But he told me he needed to do another one himself. But since yours is recent it might be OK.

He should be able to take your word for the chicken pox, but I'm not sure if every CS will do that.

But other than that, according to your age, you should be all set with the shots.

I believe you will still need to have the blood work and the TB test done by the Civil Surgeon so be prepared for those to increase the cost of your test. Everything else seems good. Good luck.

That is what I thought at first (a few months ago), but the different nurses I spoke to all confirmed that blood work and t.b. test can be done by a separate doctor, as long as I provide the c.s. with the results (paperwork).

Adjustment of Status

11/03/10 ------- AoS (I-130/I-485) Package mailed out (Priority Mail)

11/07/10 ------- AoS Package received and singed for

11/10/10 ------- NOA1 received for I-130, I-485 and I-765 (emails)

11/12/10 ------- NOA1 received for I-130, I-485 and I-765 (hard copies)

11/12/10 ------- Touches on I-130, I-485 and I-765

11/19/10 ------- Biometrics appointment letter received

12/06/10 ------- RFE for I-693 (I think the issue is that it was not signed. Called USCIS and will receive a letter in a few days explaining)

12/13/10 ------- Biometrics done

12/16/10 ------- EAD card in production (email)

12/20/10 ------- Received "Letter of Explanation" for RFE (Service Request to expedite my case. Called USCIS and was told to ignore that and just send in response to RFE.)

12/22/10 ------- Touch (Email for Post Decision Activity on EAD saying that a letter of approval has been mailed out)

12/24/10 ------- Received EAD in the mail

12/27/10 ------- Applied for SSN

12/31/10 ------- Received Interview letter

01/03/11 ------- Received SSN card in the mail

01/07/11 ------- Mailed out response to RFE (I-693)

01/15/11 ------- Email confirming USCIS received RFE response

01/31/11 ------- Approved!

Pre-Adjustment of Status:

2006 -------- Met Online

02/07 ------- Visited him in the U.S. for what was suppose to be a few weeks (Came in with birth certificate and health card. Health card expired a few months after)

08/07 ------- Decided to get married because we didn't want to be apart (in the U.S.)

10/10 ------- USCIS Medical Done
