Hey we are trying to figure out what my fiancee will need once she gets in country. She has her MMR shots and we have proof of it. We aren't sure about other shots.

Does anyone know what shots she would need? She's 27 and healthy. She only has an allergy to cats and atopic dermatitis.

What shots are typically required? There is a list on the embassy's website with about 14 shots, but there's no way she should need all of them.

So if she has already gotten the medical check while she was in Japan, after getting the remaining shots we just need to get the DS2035 form and have Civil Surgeon fill it out and send that in with our AOS application?

And it's hard with the guides because in the link that you gave me and this topic http://www.visajourney.com/forums/topic/260605-where-do-you-get-i-693-from/ I was given another website/adobe file that makes it seem like she wouldn't need Heb B, Haemophilus influenzae type B, or polio vaccines...there's a link in one of the last posts.

So it's really confusing...Sorry!

I think you're still mixed up. The DS-3025 would come from the PANEL PHYSICIAN who gives the medical exam in a foreign country for a visa. If she had her exam, they should have given her that form showing her history of immunizations.

Once in the US, the doctor who would be approved for immigration (AOS) purposes is called a CIVIL SURGEON. The form he uses is called I-693. So you can have a civil surgeon complete the immunization section of the form I-693. He would refer to the DS-3025 to know what shots she got in Japan. He would refer to any documentation of shots she got after arriving in the US like at a private clinic or the health department. Or he could give shots himself. Then he compiles all the documentation of shots onto one form (I-693), signs off on it, and seals it up for you to submit with your AOS.

It sounds like you didn't look at the links given you above. This document has a chart on p. 16 showing what is needed by the different age groups.

http://www.cdc.gov/immigrantrefugeehealth/pdf/2009-vaccination-technical-instructions.pdf

A 27 year old will need:

MMR (Mumps, Measles, Rubella)

Td or Tdap (Tetanus/Diptheria or Tetanus/Diptheria/acellular Pertussis) Those are adult versions of DTP for kids

Varicella or a history of having varicella (chickenpox)- It could be marked "VH" by varicella on the DS-3025

That's it. Read what the official documents tell you in that link and you will get the correct info. Don't depend on this forum because you have been given some very different answers.

it's a personal situation. Obviously you had risk factors that required a Hep B. I did not need one and almost everyone I know didn't either... there is the odd exception of course.

Here is a table: http://www.cdc.gov/mmwr/PDF/wk/mm5901-Immunization.pdf page 2

Weird!! it's not age appropriate for me (I'm 26, almost 27 :D.. my first bday here :D) Well some CS's will do anything to get money out of people. Won't hurt you anyway just strange 'cause unless you have a risk factor it's not needed.
