Hello,

I am filling out form G-325A. My wife was divorced over 10 years ago, but used her former husband's last name after the divorce for all identification purposes. Should I put this as her maiden name, or use her last name at birth (on this and any other forms that ask for it)?

Her maiden name is the name she was born with. I was in the same situation and what I did was on the 325A under "All Other Names Used (include names by previous marriages)" I just listed my maiden name and then after a few spaces the name I had been using from my first marriage. I had no problems at all! Good luck!

This advice will serve you well as you proceed. "Read carefully, interpret literally and answer accurately." When asked for maiden name, give the maiden name for anybody who was ever a maiden. When asked for all other names used, give all other names used. My preferred method of listing other names (when there's room to do it this way) is Jane Mary MAIDEN, SMITH, JONES, CURRENT. with all family names in all caps in the order used. The above reflects CURRENT as the third married name after two previous marriages. Adjust as needed. For space problems, drop the first and middle.

You are not likely to get an RFE on this, as the correct information is available throughout the package. On another note, you should be careful to follow threads that are more recent, to ensure the information is up to date. This thread is over three years old.
