Hi, our petition for fiance visa has been recently approved and now we're on the process of scheduling an appointment with the Philippine Embassy.

My fiance lives in the US and is in active duty, while I am here in the Philippines. After filling up the Form I-134, does he need to file it anywhere in the States or just have it notarized and send it to me? I'm planning to set my interview on May 24 which is roughly 3 weeks from now.

He does not have to submit anything else directly to the government, nor does the I-134 need to be notarized. He just has to ship it to you along with the required supporting evidence. I'm inclined to say more evidence is better, but some opt to present the minimal amount necessary. For me, I handed over at the interview: last three year's worth of tax returns and W-2's, letter from bank, letter from employer, last six months of pay stubs, etc.

He sends it to you but you'll be dealing with the US Embassy, not the Philippine Embassy.

I seldom advise an employment letter. It does little that pay stubs won't do. Any evidence of active duty status, rank and pay is generally sufficient evidence of current income. The petitioner will know what they have indicating rank and pay.

It absolutely positively must be the ORIGINAL with his original signature. It should not be scanned nor faxed. It is a signed affidavit.

Help!

My fiance has sent it to me already but he printed it in 2 separate pages. also, he checked "do not intend" in question 11 where they asked if he intends to make specific contributions to support me. Since he thought he wasn't going to make "specific contributions" anyway. my interview's in one week and there just isn't enough time for him to send me another form.

The form is 2 pages, and can be printed on two separate sheets. It doesn't have to be printed on both sides of the same page.

Question 11 is largely irrelevant for a K1, but some consulates are picky about how the question is answered. I haven't heard about it being a particular problem at the Embassy in Manila, though. In general, the supporting evidence is more important than the form itself.
