Okay so I attended at a CS who knew about the immunisations only so I thought YAY.

I took my form and left for a couple of days while the CS's wife filled it in like it was supposed to be. Here they are:

I collected it and the girl asked me to check the form. I noticed my A number missing on the immunisation page and filled that in (so I handwrote it), but failed to check the forms any closer. Since I got a copy I was able to notice the following:

1. the front form is the new form (that I completed), the last 2 pages (that the CS wife filled in) is the OLD form... so the "revised" date is different and there's no barcode. The old form is still valid though.

2. the tDap has an X in the "completed" box, but the HPV and MMR don't have the X in the "completed" box

Aside from that it's fine. My immunisations are up to date I'm just concerned that the fact my pages are different, and there being no X's will be an issue.

Opinions?

Someone must have an opinion. I know it's hard to know facts, but I'm after opinions. If it was you, would you go to the CS to get it fixed FIRST, or risk it?

Also, I'm moving to Iowa in less than 2 weeks so if it IS wrong I'll have to pay another CS, or get this CS to send me another without charging me... chances of which I think are small if I'm not bugging them in person.

i belive if they used the old form they have two options for the hpv, and that is to either leave it blank or write n/a since it is no longer required.

As for the MMR, it should have an x if you received all 3 shots, one as a child, another one and then a third as an adult. What does your vaccination book say?

Yeah it has all shots. The stuff written is fine, just the X is missing. She claims it's fine that she's done plenty, but I don't want to get an RFE because of a missing X!!! lol. Do you think USCIS will just ignore it because the dates are all fine and they'll just realise the CS's wife is a moron?

For those that are interested I ended up getting a new form completed.

Not only were the last 2 pages in the old form where the front page was the new one, it didn't even have the barcode it was supposed to have so it was some strange version of the old form.

I filled in the old form because it had space for the HPV and because the form was still considered valid except the version from USCIS had the barcode. I copied all the info from the form they filled in into the old form and printed it. i took it in for signature and that was it. Well they checked it first but then that was it.

It was a pain because the lady the stuffed it up wasnt there but we dealt with the girl that we dealt with the first time we were there who was very nice. We actually were very happy the old woman wasn't there because we're sure she would have told us it was "fine" as it was. I'm trusting that they re-sealed the good version and didn't just give me the old envelope back but I suppose we'll find out... i should have marked it or something.. seriously they are/were that dodgy.

Either way, very happy we don't need to deal with them anymore! :D

06-04-2007 = TSC stamps postal return-receipt for I-129f.

06-11-2007 = NOA1 date (unknown to me).

07-20-2007 = Phoned Immigration Officer; got WAC#; where's NOA1?

09-25-2007 = Touch (first-ever).

09-28-2007 = NOA1, 23 days after their 45-day promise to send it (grrrr).

10-20 & 11-14-2007 = Phoned ImmOffs; "still pending."

12-11-2007 = 180 days; file is "between workstations, may be early Jan."; touches 12/11 & 12/12.

12-18-2007 = Call; file is with Division 9 ofcr. (bckgrnd check); e-prompt to shake it; touch.

12-19-2007 = NOA2 by e-mail & web, dated 12-18-07 (187 days; 201 per VJ); in mail 12/24/07.

01-09-2008 = File from USCIS to NVC, 1-4-08; NVC creates file, 1/15/08; to consulate 1/16/08.

01-23-2008 = Consulate gets file; outdated Packet 4 mailed to fiancee 1/27/08; rec'd 3/3/08.

04-29-2008 = Fiancee's 4-min. consular interview, 8:30 a.m.; much evidence brought but not allowed to be presented (consul: "More proof! Second interview! Bring your fiance!").

05-05-2008 = Infuriating $12 call to non-English-speaking consulate appointment-setter.

05-06-2008 = Better $12 call to English-speaker; "joint" interview date 6/30/08 (my selection).

06-30-2008 = Stokes Interrogations w/Ecuadorian (not USC); "wait 2 weeks; we'll mail her."

07-2008 = Daily calls to DOS: "currently processing"; 8/05 = Phoned consulate, got Section Chief; wrote him.

08-07-08 = E-mail from consulate, promising to issue visa "as soon as we get her passport" (on 8/12, per DHL).

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).

Not to add injury to insult but on the second page, shouldn't the box that reads "Vaccination history complete for each vaccination, all requirements met" should be ticked off instead of the first box?

Nope. Because I haven't had every single shot because of the age requirements. Because there are ticks in the "not age appropriate" box you'll see at the top of those collections of boxes that it says a waiver needs to be applied for. You're only supposed to pick one box :)

the only time that box SHOULD be ticked is if every single shot has been given. Valid question though :D

Thanks for your response...my husband hasn't had every shot because of his age requirement yet our CS filled out the vaccination completed box... I was just curious... and he has spots ticked off for age not appropriate...

Different CS's seem to do this different ways.

Here are the form instructions: http://www.cdc.gov/immigrantrefugeehealth/pdf/2009-vaccination-technical-instructions.pdf page 13.

Here is the part about "not age appropriate". When ticking box:

"Applicant may be eligible for blanket waiver(s)...

Completion of a vaccine series is not required to conclude the medical examination, because such a requirement would require multiple visits to a civil surgeon and could lead to unnecessary delay in the immigration process. If any of the boxes under the “Not Medically Appropriate” heading was checked, the “Applicant may be eligible for blanket waiver(s) as indicated above” box must also be checked. This box will probably always be checked because some vaccines may not be age appropriate for the applicant"

I bolded that last section. :)

Here's the part about ticking the complete box:

"Vaccine history is complete for each vaccine.

If the applicant has met the vaccination requirements, i.e., completed the series for all required vaccines, the “Vaccine history complete for each vaccine, all

requirements met” box must be checked."

Again I bolded that part. It seems to me like you shouldn't have that part ticked but I doubt it matters a great deal.
