i received an email from cris saying my case has been transferred to CSC too yay alhamdulillah thank GOD the most merciful :energy:

Hi everyone,

I'm just filling out all my AOS forms and I have some trouble finding what I have to attach to the I-131 form, Application for Travel Document. Can anyone help me? I know I have to include two passport size photos but do I need any more copies of anything else?

I also have a question about the I-693A do I have to provide that form to the civil surgeon or do they have it in their office. I could not locate it anywhere in this website so I don't know what to do.

Under the tips for helping you fill out all the documents it states, "I-693, Medical Examination of Aliens Seeking Adjustment of Status. (if you already have this from your overseas visa interview submit a copy that medical check form......" what is "that medical check form"? did anyone submit anything as proof, a copy of the receipt or anything else?

Best thing to do is look in the main aos thread for the UK and on I think page 6 is a full list of everything you will need

I just sent my medical form in that I received at my medical in the UK as they signed the waiver box I did not get it transcribed, seems ok as I got a e-mail saying my ead and ap were approved yesterday and my aos has been transferred to California did not get a ref for only sending in the copy of the medical from the Uk and not sending in the I 693

normally they should send my application to Vermont service center but as you can see they sent it to CSC.

your case is still at the National service center at chicago, so don't lose the hope to be transfered.

So, if anyone who has changed your last name after getting married wants to try a walk-in with any of the Application Services Center, make sure to also bring your marriage certificate! Good luck, everyone!!!

For the Visa we coudl call the nvc.

sent my AOS august 17 and got the hard copy of NOA1 august 24.Am gonna have my biometrics app on sept 23,unfortunately i got RFE,huhuhu!!!they need the pay stubs or other documentation of pay for the previous 6 mos.

I just checked and got emails today to say that both mine and my daughters EAD and AP cards were approved and ordered on Friday...i'm so happy, I can't wait to get a job!!!

no it has the horrrible one I sent them, the biometrics pic was not great either that is for the aos so I will have a matching pair of ####### pics hahaha just got a mail from CRIS that they sent my ead yesterday, cant wait for my back to get better so I can look for a job
