Hi everyone... I came here in the US on a K3 visa. I am submitting my AOS in a few days. My question is do I need to submit I-693 when I already have my DS-3025? I have read before that if I have my DS-3025 and its less than a year from the time of medical exam that I don't need I-693. But I read here just today that I still should go to a doctor to fill out the form I-693 and there will also be a fee for that... Please enlighten me about this matter...thanks...

If you have not completed your vaccination shots then u have to go to a civil surgeon to acquire I-693, of course, u will need to complete your shots. I did not submit my I-693 along with my AOS application but I will bring my completed form on my interview because it was stated in my interview notice. Some were able to pass the AOS application without submitting I-693 though... it depends...

u need to have it transcribe by a certified civil surgeon in your area. only CS will determine what are the additional vaccines that u need to get.

My husband only submitted the DS-3025 and received his green card last month without an RFE for the medical. Keep in mind that you had to have every shot required recorded on that form. If you are lacking anything (like some people can't get HPV in their country) then any shots you got after entering the US, must be recorded by a Civil Surgeon on an I-693 along with the whole record of shots recorded on you DS-3025.

Here's a 2009 list http://www.cdc.gov/mmwr/PDF/wk/mm5753-Immunization.pdf where you can see if you've had everything. Also look at your DS-3025 and see if everything is documented with a date or and check for not age appropriate. Many on the list are only for children so check the age appropriateness.

I was 28 years old when I had my vaccination, so I assume I am not given HPV. I have my DS-3025 and so far I see all the check marks in the appropriate columns... I hope I don't need to submit I-693 anymore even on the interview... i will submit DS-3025 for now...i will wait what it will say on the interview notice...

Many people only sent a copy of the vaccination worksheet that was given to them after their medical before their visa interview and never had a problem with AOS. Keep in mind that you may receive an RFE for it but all you would have to do is get a civil surgeon to fill out the I-693. You can use Dr. Arnold from TX, it'll be a lot cheaper. You can find the doctor's info if you do an extensive search here on VJ.

I don't think you'll get an RFE. We fell under the new rules with a medical exam after June 1 when all the talk of transcribing onto an I-693 being required, even if the shots were totally complete abroad. We basically tested the system and waited and waited for an answer....RFE or GC?? Now I now know of ten or more others who did the same and got green cards without an RFE. It will depend on what your interview person says in the end, but I would certainly argue the point that you have a document by a US chosen panel physician who certified you had all the shots and a civil surgeon would only be taking his word for it and charging you to put it on a new form.

Technically, EVERYBODY MUST include a for I-693 completed by a USCIS-approved Civil Surgeon.

Those who have completed a physical within the last year also need to submit an I-693, but only the vaccination section needs to be completed, not the whole form.

That's what's technically required. Now, as to what you can get away with.

You may be able to get AOS approved by just sending the DS-3025. But lately less and less people are getting through.

For my wife's AOS (July 2008), we only sent her DS-3025 and she got approved in December. For our daughters, we also only sent the DS-3025 (Feb 2009) and received an RFE for medical in April.

So, if you don't mind a delay (2-3 weeks), try sending the DS-3025 only.

Non-immigrant visas like K-1s do not require vaccinations to be completed at the original medical and that is why the additional vaccination information is required for AOS. Immigrant visas should require all vaccinations to be completed at the first medical, and therefore should not need additional vaccination info.

I'm curious about this subject because it's been tops on my mind since October. Your wife entered the US before the deadline, so she would have had a medical prior to June 1. So she wouldn't fall under the "new" rules anyway. I'm wondering about the daughters. Did they require additional shots or boosters or was it explained that only the form was wrong and nothing else. I still always read threads about medical RFEs, but I haven't seen any that were only about the form. There's alot where the whole medical appeared to be lost or didn't make it to match up with the AOS file. Where are you finding these reports because I'd like to read them and must have missed them in my looking. What I find is reports of not sending the I-693 and getting the GC without RFE.

I wonder if it's because the CDC makes up the list based on drugs available in the US. Probably not every country in the world has those immunizations so some would be impossible to get until the person arrives in the US. Just a guess.

I disagree. The I-693 instructions are very clear. You aren't required to have another full exam, but you are required to have a civil surgeon complete parts 2 and 5 of I-693 (see below).

What if I am a K nonimmigrant visa holder and already had a medical exam overseas?

C. You received a medical examination prior to admission, then-

a. You are not required to have another medical examination as long as your Form I-485, Application to Register Permanent Residence or Adjust Status, is filed within one year of your overseas medical examination.

b. You will, however, be required to complete Part 1, Information about you, and submit the vaccination section of Part 2 with your adjustment of status application. A designated civil surgeon must complete the vaccination section and Part 5, the Civil Surgeon's Certification.

But the problem is not the clarity of the instructions, it's the inconsistent enforcement of the policies and procedures. Some people get by while others in the exact same situation, get RFE's.

As for my daughters, it said that the file did not include the medical exam results, so it needs to be redone. It wasn't just for the form. But it clearly said that they needed a completed I-693. I think they "misplaced" the medical results and made us redo it. Luckily, the whole exam only costs $80 each.

If the beneficiary's vaccinations were COMPLETED as part of the visa process, IT IS NOT NECESSARY TO INCLUDE FORM I-693 IN THE AOS SUBMISSION. All your medical information is enclosed in the brown envelope which was handed in at the POE. (this is what I had in mind) The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received. If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693.

"Q: Do I need to complete another full medical to adjust status?

Per Section 1245.5 in the Code of Federal Regulations, a medical examination shall not be required of an applicant for adjustment of status who entered the United States as a non-immigrant spouse, fiancée, or fiancée of a United States citizen or the child of such an alien as defined in section 101(a)(15)(K) of the Act and §214.2(k) of 8 CFR chapter I if the applicant was medically examined prior to, and as a condition of, the issuance of the non-immigrant visa; provided that the medical examination occurred not more than 1 year prior to the date of application for adjustment of status."
