Did anyone have a copy of their I-693 from the CS?

Hi all.

Has anyone been given a copy of the I-693?

Are we even supposed to have a copy of it from the Civil SUrgeon?

I'm supposed to make an infopass appointment. I was just wondering If I ever have to mention it, coz that's where I found an error with my I-693...

AOS Timeline:

12/10/2004 - Filed AOS to Newark

9/27/2005 - Case transferred to CSC

12/2/2005 - Notice Date of RFE for Medical

2/1/2006 - Sent back Medical RFE docs to CSC

2/2 - CSC received RFE docs

2/7 - "Touched"

2/8 - "Touched"

3/9 - "Touched"

3/10 - "Touched"

4/20 - Another Medical RFE for properly filled out I-693

5/10 - sent out RFE docs

5/19 - case APPROVED!!!!

5/26 - "error-free" Green Card in the mail!

We also asked for and received a copy.

I asked for a copy and I got one as well as the one in the sealed envelope...

I think most people get a copy for their own records....

In case people do not get it, they should -- they should ask for one.

09-02-2005 Applications for AOS, EAD, and AP received by MSC

10-21-2005 AOS fingerprint notice for 12-08-2005

11-07-2005 AP approved

12-05-2005 Infopass appt at San Jose office for interim EAD -- Refused, because it is already approved by MSC on 11-07-2005

12-07-2005 Attempt at interim EAD at San Francisco office -- no go. Back to San Jose, where CSO (chief station officer) tells they will contact MSC via email to request permission to issue interim EAD

12-08-2005 Biometrics for AOS and EAD. Having no EAD appt letter was no problem (used EAD NOA)

12-15-2005 EAD arrived in the mail

12-24-2005 Received interview letter; interview scheduled 03-01-2006

01-28-2006 Received replacement SSN card in married name (5 wks since application)

03-01-2006 AOS interview -- approved; received stamp in the passport

03-13-2006 Green card arrived in the mail

Well, I asked and I didn't get one....am I screwed now? The cs wouldn't give me one saying that he was not allowed to do so...

04/17/2008 - Biometrics scheduled

05/19/2008 - received I551 extension stamp via INFOPASS

02/12/2009 - APPROVED

02/21/2009 - GC received...no mistakes...valid until 2019

I asked as well - and was told no, they were only supposed to provide a copy of the vaccination work sheet - the rest had to be in the sealed envelope - no copies provided. I did get a copy of the vaccination supplement without asking, though, when I got that done here.

I was also told that I shouldn't get a copy of the I-693 medical form by the Civil Surgeon. I was given a copy of the rest of the other forms like the vaccination supplement, xray report, bloodwork, etc.

rch99, I'm referring to the I-693 filled out by a Civil surgeon here, coz I got an RFE for a medical exam, and after 2 months, another RFE for a "properly completed" I-693 and an evidence of my xray report, whch was actually included in the first RFE docs I sent them.

Looking at the site, downloading the forms, there are actually 4 forms - CS copy, USCIS copy, applicant's copy, and for the physician or health department. So, I guess, maybe, applicant's should have a copy of the form. But, I guess, most of them, don't.

I didn't ask for a copy, but they gave me 3 copies. one for health department, one for myself, and another one for... um I don't know. but luckily, from the copy, I found error( my name was wrong!!), before I filed.

I'm talking about I-693 from CS here in Colorado.

I printed 4 pages out and took them there, and they only needed 1st page, and they made photocopies of everything(I-693, Vac sheet, HIV result) and gave to me.

2009. 5.15 Check Cashed

2009. 5.18 Recvd NOA1, NOA date is 5/13/09

2009. 6.16 Biometrics done

2009. 9.14 Interview - Passed!

2009. 9.21 Oath

Finally done with USCIS... FOREVER I hope

I-693 or vaccination supplement? they are 2 different things created in 2 different countries.

No. you don't need another.

No. you don't need another.
