Oh really. It seems the directions in the AOS specfically ask for a I693A.... Then what happens when they get a RFE asking for I693A?

Not to take any business away from Dr Arnold but if you have all the required vaccinations already listed on the DS-3025 then you already have what you need.... Only if you need to add some vaccinations that you may have given subsequent to the issuance of the DS-3025 will you need this done....

Sorry, I haven't read all threads/topics...why Dr Arnold is so famous in VJ? Actually where is she?

She's the only CS that will work on your I-693A for $10 without the need of physical appearance at her clinic. You can just call her, fax or mail the vaccination records and she'll mail you back the completed medical supplement for your AOS. I believe her clinic is in Texas.

I am in Washington State...Pacific North West...I would like to know if the doctor that will put this information about my vaccine (DS3025) to the other form..need to be a doctor indicated by the USCIS?

Or any doctor can do this?

Since you have the DS 3025, anecdotal experience indicates you do not need this other form. However, if you choose to prepare the vaccination supplement it has to be performed by a Civil Surgeon as authorized by the USCIS, so not any old doctor will do.

FYI I got a return call from Dr. Arnold's office today. The fee is $20 if you want it mailed priority. You can still pay $10 for regular mail.
