I went to my interview in US embassy in Tunisia in January 17 and in January 23 my visa and my passport were delivered without any sealed packet and no medical exam with it 
I called the embassy and they told me it's new policy and typicaly normal.. however so many applicants still get the sealed packet along with their visas
what should I expect from the custom borders in the port of entry ?

They’re using an electronic system now. No need to be anxious about anything.

On the visa I believe it will be marked "Electronic File" or something or that nature.

I got that letter in November 2017 then in January 2018 I got my interview letter so it's about three month waiting and my interview was in February 2018 and in 15 March 2018 I joined my husband in America finally so u have to be patient little more I know it's so frustrating waiting and the nvc always know nothing when u call them just saying wait more. I wish u all the best 

Calling the embassy is not a good Idea ......don't seem frustrated in their eyes...

Yes, you can schedule the medical appointment once you receive your interview appointment notice. I'm not sure how long Guatemala is taking. My wife (in Romania) received her interview notice 2 weeks after being "documentarily qualified". Again, not sure about Guatemala, some other users more familiar with that region can add their experience.
