So happy to say, I received my NOA2s approvals for both my EAD and AP today!!! Two separate letters for each, although the EAD letter did say that my card will also show that I am approved for AP, so I'm not sure if I will receive the combo card or the EAD card and then the AP document separately.... I will let you know when I receive the card which I am assuming will be next week! 
 
My online status still reads "Case Received" for both EAD/AP, so the online tracker doesn't seem to be working accurately!
 
I sent my package off on 09/29
USCIS received on 10/02
NOA on 10/11 
Bio Appointment 11/06
NOA2s 12/09
 
So 2 months and 7 days to get approved, cannot believe it!

I know of some that USCIS would not follow the name as shown on the I-485 and kept the middle name as shown on the original visa, but no RFE for that.  Like said naturalization allows you to legally change your name.
 
You are stressing too much about this.
 
You can still complete ROM using traditional naming format on the form.  The form you complete for ROM .. THAT is what will show up at PSA when you request a copy of your marriage certificate later.  Not a copy of your marriage certificate.   And THAT will allow you to get your Philippine passport in your traditional married name.

I understand that but since we have to go through immigration and immigration guys want proper documentation and so we can confuse ourselves and the immigration. My suggestion for AOS is go with your Passport Name. Good Luck  

Hi there,
Please fill the applications with the way your name needs to be in the GC and other cards and send it in (fill it without the hyphen, which really is not a problem).
1. The day you go in for your bio-metrics, carry the marriage license and your passport, so the officer (who sits in an office and not the bio-metric person) will review the document and sign off the name change.
2. The bio-metric person will then enter the details the way you want the documents to be, make you verify it twice and then will hit submit and ask you to sign on the electronic pad (you can use your new signature on the pad and this time duration will be enough and more to master your married signature)
As for your SSN
1. you will now get the SSN only in your maiden name.
2. Once you receive your GC (not just the EAD / AP combo card but the GC) go back to the SSA and change the name on your SS card. Once the GC is in, they will change your name and also the status on the SS card.
Dont worry about the names in the marriage certificate. That is proof that you (in your maiden name and of sound mind) did marry your husband.
As for the old middle name, yes fill out the forms like this for example;
If your old name was Nancy Maria Smith and now you want your name to be Nancy Smith Borden, fill the forms like...
1. a. Petitioner's Name: Nancy Smith Borden
1. b. Other Names used: Nancy Maria Smith

I would need to double-check the paper work, but I think we reported her name using the Philippines naming convention. She also was able to get her passport renewed with that name as well. We have gone to the Philippines since then and we had no problems with the middle name being different as we omitted it from the plane tickets. We also borught along a copy of her birth certificate and a copy of the marriage certificate just in case, but neither were needed fortunately.
 
My wife is in the process of applying for citizenship and as part of that, she is changing her middle name to follow the Philippines naming convention, to avoid any potential issues in the future. Unfortunately, we will then need to update her middle name on any accounts and IDs that have her current one. I think it will make things easier when she eventually applies for dual citizenship and we eventually buy a townhome/condo at some point. A lot more to look into around that...
