The more evidence items the better. Though I don't have specific inside knowledge, if you provide them more than one evidence item they scrutinize each a little less. Also from what I understand the more assets you have in deposits in the bank the less dollars of income are important and vise versa.
If was self employed I would make sure I supplied the back officer statements (my experience with banks are if you are persistent with them they will provide bank officer statements, but the results of these statements may vary) and tax transcripts. Then provide self-employment the best you can.

Yes. need a form for each person. Even minors from my understanding. Later your beneficiary will be able connect the them in a traveling together section.

Do mind sharing which item of evidence you provided? I provided Bank officer statements, Employer Verification of employment and wages, tax transcript and birth certificate. Do you think these item will do the trick?
