We were married yesterday and already have the certified copies of the marriage license. We are ready to adjust status.

 

After reading the pinned posts in this forum, my understanding is that we should go ahead and send in a partial I-693 to give us the best chance of avoiding delays and issues. Does this mean we also attach the DS-3025 to the I-693 when we mail the package? I would love to avoid this extra I-693 form if at all possible because who knows what confusion may occur if we have to meet one of these "civil surgeons". The pinned post from user Nich-Nick mentions that civil surgeons may improperly mark boxes because they may not understand the meaning of some of the boxes and that civil surgeons may try to do a full exam when that isn't needed. Sounds like a nightmare.

 

Below is the DS-3025 she received from St. Luke in the Philippines. After reading the pinned post in this forum I'm not sure whether or not it was completed fully. For example, the pinned post says two shots of MMR are required. On the DS-3025 I see one date, does that mean two were given or that another is needed? For Influenza, there is a date but also a B waiver talking about "insufficient time interval to complete series" so do we need a civil surgeon to administer a flu vaccine?

 

The original DS-3025 is double sided. A note is at the top saying not to lose this form and that its a personal copy and will be needed to present upon change of status. Does this mean we should keep this copy of the DS-3025 or does this mean we should mail it in with the I-485 ? We made copies but the copies aren't double sided, does that matter?

We have the original DS-3025 which is double sided. We also have copies of the DS-3025 which are single sided copies. So, in our copies, page 2 is on its own separate page whereas the original DS-3025 has page 2 on the back of page 1. Do you know if this matters?

USCIS prefers single sided documents. Make sure after sending them these docs you still have other copies for yourself.
